Home mechanical ventilation and bilevel PAP

Home mechanical ventilators and bilevel PAP coverage statements (applies to members age 2 years and older):

Ventilator mode non-covered scenarios: Home mechanical ventilators are not medically necessary when used only in bilevel PAP mode (HCPCS E0470, E0471) or when used only to deliver continuous or intermittent PAP (HCPCS E0465, E0466).

Medically necessary ventilator indications: Home mechanical ventilators (HCPCS E0465, E0466) are considered medically necessary for neuromuscular diseases, thoracic restrictive diseases, and chronic respiratory failure consequent to COPD in certain clinical scenarios as defined by InterQual criteria.

Stance on use of HMV/BPAP in COPD relative to PaCO2:

Additional illustrative exclusions include elastic splints, sleeves, or bandages when not part of a covered service; oral appliances for snoring; orthotic braces intended to change body shape; powered and non-powered exoskeleton devices; and personal care/comfort items (for example, television or telephone). Replacement for loss, theft, malicious damage, routine maintenance for which the owner/vendor is responsible, and items explicitly excluded by the member-specific benefit plan are also not covered.

This policy and its coverage statements do not apply to Durable Medical Equipment or supplies that are intended for use in an outpatient or inpatient facility; the policy is written to address DME and supplies provided for outpatient/home use.

Facility use of equipment should be evaluated under the facility’s applicable policies and benefit documents rather than this outpatient/home DME policy.

In the absence of a related policy or coverage indication, UnitedHealthcare applies the following guidelines in order: InterQual® CP: Durable Medical Equipment; InterQual® Medicare: Post Acute & Durable Medical Equipment; and Centers for Medicare & Medicaid Services (CMS) DME MAC guidance.
