Medical Necessity Criteria

Pediatric Gait Trainers - Functional Ambulation

Gait trainers for Functional Ambulation are proven and medically necessary when ALL of the following are met:

ALL of the following

The individual is 18 years of age or younger.
The individual has the potential for Functional Ambulation.
Documentation shows assistive devices have not been effective.

Pediatric Gait Trainers - Therapeutic Ambulation

Gait trainers for therapeutic ambulation are proven and medically necessary for treating non-ambulatory individuals when ALL of the following are met:

ALL of the following

The individual is 18 years of age or younger.
The individual is capable of utilizing and tolerating the equipment safely.
The individual requires moderate to maximum support for ambulation (i.e., handheld ambulation assist devices are not feasible).
The individual has an acquired injury (e.g., spinal cord or traumatic brain injury) or a chronic physical limitation that affects the ability to ambulate (e.g., cerebral palsy, neuromuscular disease, or spina bifida).
The individual has a physician directed written treatment plan (including frequency and duration).

Standing Systems - Medical Necessity

Stationary, mobile, and active standing systems are proven and medically necessary for treating non-ambulatory individuals when ALL of the following are met:

There is a goal of prevention of one or more of the following medical complications:
- Decubitus ulcer: Need for off-loading of a decubitus ulcer which cannot be accomplished by other means.
- Osteoporosis: Improvement or stabilization of bone density cannot be achieved with other treatment or activities.
- Contracture development: High potential for progressive contracture formation including but not limited to postoperative release of contractures.
- Compromised bowel/bladder function: Demonstration of incomplete emptying of bladder or constipation refractory to other medical treatment.

Background & Evidence

Background: A gait trainer unweights and supports the trunk and pelvis (rigid or flexible seat with lateral/posterior components) to assist ambulation, whereas a supported standing system secures an individual in a standing position and may be static (nonmoving) or dynamic (mobile or power-assisted). This policy is limited to New Jersey. The cited evidence includes systematic reviews and randomized controlled trials showing benefits for gait training and standing programs: improvements in gait velocity and gross motor function measures (Qian et al. 2023; Chiu et al. 2020), positive changes in femur and spine bone mineral density in children with cerebral palsy (Valenzuela-Aedo et al. 2024), and supportive findings for range of motion, hip biomechanics, spasticity, and dosing recommendations from Paleg et al. and Paleg & Livingstone.
