inv-01: HHD without skilled care

Covered when ALL of the following are met:

HHD without skilled care — required conditions: Individual undergoing hemodialysis or non-professional caregiver has the ability to perform and maintain home hemodialysis and has received comprehensive training regarding proper protocol; and Individual is stable on dialysis with no evidence of skilled care interventions being necessary during treatments; and Absence of complications and significant concomitant disease that would cause home hemodialysis to be unsafe or unsuitable; and Individual has well-functioning vascular access.

Covered when ALL of the following are met:

HHD with skilled care — required conditions: Individual undergoing hemodialysis or non-professional caregiver is not capable of performing home hemodialysis; and Individual is stable on dialysis and not at increased risk as a result of having the procedure performed outside a dialysis center venue; and Individual has medical contraindications to leaving home for hemodialysis; and Treatment regimen typically 3 times per week; 3-4 hour treatments (exact therapy determined by the attending nephrologist); and Individual has well-functioning vascular access.

inv-03: Clinical evidence summary

Summary of clinical evidence findings relevant to coverage decisions

Physiologic and patient-reported benefits: Frequent (including daily) in-center or home hemodialysis demonstrated improvements in left ventricular (LV) mass, blood pressure control, and some health-related quality-of-life measures (including shorter recovery time) in randomized trials such as the FHN Daily trial and related analyses.

Adverse effects and risks: More frequent or longer hemodialysis was associated with increased vascular access interventions and higher rates of intradialytic hypotension in several trials and systematic reviews.

Home HHD coverage: Home hemodialysis without skilled care is an alternative to facility-based hemodialysis for individuals with end-stage renal disease when the individual meets the policy's coverage criteria, including demonstration that the individual (or non-professional caregiver) has the ability and training to perform and maintain HHD, the individual is stable on dialysis without need for skilled interventions during treatments, there is absence of complications or significant concomitant disease making HHD unsafe, and the individual has well-functioning vascular access.

Home hemodialysis (HHD) is not appropriate for individuals who have complications, significant concomitant disease, or lack a well-functioning vascular access. The policy requires that the individual be stable on dialysis and that the person undergoing dialysis (or a non‑professional caregiver) has demonstrated the ability to perform and maintain HHD after comprehensive training; absence of these elements excludes HHD as unsafe or unsuitable. Practical suitability criteria also include having a suitable space in the home and, when applicable, a caregiver who has made an informed decision to assist. Well‑functioning vascular access is required to provide adequate blood flow for treatment and is a specific exclusion if not present or functioning properly.

Evidence from trials and systematic reviews addressing more‑frequent or extended‑length hemodialysis is limited by small samples, mixed results, and generally low to very low certainty for many critical outcomes. Several analyses did not demonstrate durable improvements in mortality or other key clinical endpoints over short follow‑up, and many studies were not powered to assess long‑term mortality. When reviewing requests for regimens that exceed conventional frequency or duration, the clinical evidence and the limited follow‑up should be considered in authorization decisions and documentation should include justification of the expected benefit for the individual patient.

The NKF/KDOQI guidance highlights that randomized trial evidence for home short‑frequent hemodialysis is lacking and that overall data regarding home long‑frequent HHD are inconclusive. Given the uncertainty about efficacy and the potential for increased harm, including concerns about mortality in some extended‑follow‑up analyses, NKF/KDOQI did not make firm recommendations for home long‑frequent HHD. This guidance supports a cautious approach when considering such regimens outside of well‑justified clinical scenarios.

Home hemodialysis is considered not medically necessary when the patient or available caregiver lacks the ability, training, motivation, or commitment to perform HHD safely, or when the individual is unstable on dialysis or has contraindications that make home treatment unsafe. Documentation of completed training and demonstrated competency is required; without this, coverage for HHD may be denied.

More frequent (>3 times/week) or extended‑length (>4.5 hours/session or intensive schedules defined in trials) hemodialysis is considered not medically necessary for patients without a clear expected clinical benefit. Systematic reviews and meta‑analyses report very low to moderate quality evidence and show no consistent improvement in critical outcomes for many patients, while also noting increased vascular access procedures; therefore, such regimens require specific justification based on patient‑level factors and evidence of expected benefit.

Outcomes in studies of more frequent or longer hemodialysis are heterogeneous: some trials report physiologic or patient‑reported improvements (for example BP control or LV mass), but overall effects on mortality and other critical outcomes are inconsistent.
