Coverage Criteria for TMJ Treatments

Medically necessary nonsurgical treatments

The following nonsurgical services are proven and medically necessary:

Nonsurgical proven therapies: Arthrocentesis; Intra-articular injections of corticosteroids; Trigger point injections; Physical therapy; Occlusal splint (stabilization and repositioning splints)

These nonsurgical services are listed in the policy as proven and medically necessary for TMJ disorders.

Surgical treatments subject to InterQual criteria

Certain TMJ surgical services may be medically necessary when InterQual criteria are met.

Coverage logic for non-surgical TMJ treatments

Covered when ALL of the following are met (policy-level guidance inferred from summarized guidance statements and evidence):

Conservative-first approach: Conservative, reversible, evidence-based therapies (education, occlusal splint therapy, physical therapy/manual therapy, exercise) have been recommended as initial treatment for TMD and should be attempted prior to more invasive interventions.

Based on AADOCR and AAOMS statements recommending initial use of conservative reversible modalities; natural history and trial data support initial conservative care.

Minimally invasive procedures (arthrocentesis, intra-articular injections of corticosteroids or hyaluronic acid, needling therapies, trigger point injections) may provide short- to intermediate-term pain and/or mouth-opening improvement; selection should be individualized and supported by prior conservative therapy and clinical documentation.

Systematic reviews and network meta-analyses report variable quality (very low to moderate) of evidence for these interventions.

Jaw mobility devices: Jaw mobility mechanical stretching devices are considered unproven due to insufficient quality evidence, low adherence, and inconclusive or unfavorable outcomes; such devices may be non-covered or require strong justification.

The policy lists certain devices and appliance strategies as unproven and not medically necessary for treatment of temporomandibular joint disorders. Specifically, jaw mobility mechanical stretching devices (for example, TheraBite® and Jaw Dynasplint®) and multiple occlusal splints (such as use of daytime and nighttime splints or simultaneous maxillary and mandibular appliances) are included on the unproven/not medically necessary list due to insufficient evidence of efficacy.

The policy states that epigenetic appliances (for example, Homeoblock™, DNA® Daytime/Nighttime appliance, Advanced Lightwire Functional appliance) are considered unsupported because a review of the medical literature did not identify quality evidence to demonstrate their efficacy or an effect on health outcomes.

Conservative therapies include NSAIDs, soft diet, jaw rest, moist heat, physical therapy, occlusal splints, muscle relaxants, antidepressants, and intra-articular injections when indicated.
Documentation should describe duration, frequency, and clinical response to conservative therapies and justify escalation to more invasive interventions.
