Coverage Criteria for Sealants, PRR, and Biomimetic Products

Sealant Indications

Covered when ALL of the following are met

Sealant Indications: Non-cavitated carious lesions

From policy indications.

Sealant Indications: Caries prevention in pit and fissures on permanent molars

From policy indications.

Sealant Indications: Caries prevention in primary molars that are expected to have a reasonable period of retention

From policy indications.

Preventive Resin Restoration (PRR) Indications

Covered when ALL of the following are met

PRR Indications: Restoration of pit and fissure carious lesions contained to enamel

For moderate to high caries risk individuals.

PRR Indications: Patient assessed as moderate to high caries risk

Indication limited to individuals at moderate to high caries risk per policy.

Sealants are not indicated in the following situations: extrinsic staining of pits and fissures, in the presence of rampant caries and multiple interproximal lesions, or for cavitated carious lesions. Preventive Resin Restorations (PRRs) are not indicated when a sealant is clinically indicated, when no caries is evident in pits and fissures, or for carious lesions that extend into dentin. Additionally, biomimetic hydroxyapatite/enamel-regeneration products are not indicated due to insufficient evidence of efficacy.

Dental sealants are thin, resinous coatings applied to the occlusal surfaces of posterior teeth to fill pits and fissures and help prevent accumulation of bacteria and food particles. Sealants may be used for caries prevention on permanent and primary molars and can help arrest progression of incipient (non-cavitated) occlusal carious lesions. Preventive Resin Restorations (PRRs) combine a restorative filling with a protective sealant barrier for early lesions that are contained to enamel in moderate-to-high caries risk patients.

Procedure Description and Documentation Requirements

Dental sealants: teeth must be isolated from saliva contamination, cleaned, and prepared with a mild acid etch to aid adherence. The tooth is dried, sealant material is applied and either light-cured or allowed to self-cure. Preventive resin restorations involve removal of early decay (limited to enamel/incipient lesions that have not progressed into dentin), restoration of the area, and sealing of grooves to prevent further caries. Hydroxyapatite regeneration medicaments and biomimetic products (for example, self-assembling peptide P11-4) are emerging adjuncts for enamel remineralization; documentation should clearly describe product used, indication, and clinical rationale.
