ALL of the following

Tooth previously received root canal therapy (non-surgical endodontic treatment)
Persistent or recurrent signs or symptoms of endodontic disease (eg, pain, swelling, sinus tract)
Radiographic evidence of continuing or new periapical pathosis consistent with endodontic failure
Restorable tooth with adequate coronal seal or able to be re-restored after retreatment
Reasonable expectation that retreatment will resolve the disease and retain the tooth long-term

This policy provides clinical guidance on indications and explicit non‑indications for a range of pulpal and non‑surgical endodontic treatments. It covers procedures intended to preserve pulp vitality (for example, direct and indirect pulp capping, therapeutic and partial pulpotomy for apexogenesis) and procedures for treating necrotic or irreversibly damaged pulps (including pulpectomy, endodontic therapy, apexification/recalcification, and regenerative endodontics). The guidance differentiates between primary versus permanent teeth, addresses immature apices and the role of interim or emergency procedures, and specifies circumstances where treatment is not indicated (for example, non‑restorable teeth, inadequate bone support, advanced or untreated periodontal disease, or situations where the pulp space is required for final restoration).
