Coverage Criteria for Prefabricated Crowns

Medical necessity indications

Covered when ANY of the following clinical indications are present (documented in the dental record):

Primary indications: Restoration of teeth with more than two surfaces affected with carious lesions, or where extensive one- or two-surface lesions are present

High caries risk indications: Large or multi-surface cavitated and non-cavitated carious lesions in documented high caries risk children; high caries risk must be thoroughly documented in the dental record (examples include: mother or primary caregiver with active caries; white spot lesions or enamel defects; visible caries or previous restorations; sub-optimal systemic fluoride intake; frequent exposure to cavity-producing foods and drinks; individuals with special health care needs; low socioeconomic status; xerostomia; more than one interproximal lesion)

Provider must document specific risk factors in the record.

Developmental and structural defects: Developmental defects (e.g., hypoplasia, hypocalcification, enamel hypoplasia, amelogenesis imperfecta, dentinogenesis imperfecta)

Interproximal and pulpal therapy indications: Interproximal caries extending beyond line angles; following pulpotomy or pulpectomy

Space maintainer and fractured teeth: Restoration of a primary tooth that is to be used as an abutment for a space maintainer; intermediate restoration of fractured teeth

Tooth surface loss and hygiene limitations: Restoration and protection of teeth exhibiting extensive tooth surface loss due to attrition, abrasion, or erosion; individuals with impaired oral hygiene in which breakdown of intra-coronal restorations is likely

Isolation limitation: When the tooth cannot be effectively isolated for amalgam or composite restorations

When indicating a prefabricated crown for large or multi‑surface lesions, the provider must document the patient’s high caries risk and the specific risk factors in the dental record. Acceptable documentation includes clinical findings and history that support high risk (examples listed).

Documented high caries risk with one or more of: mother/primary caregiver with active caries; visible caries or previous restorations; white spot lesions or enamel defects; sub‑optimal fluoride exposure; frequent exposure to cariogenic foods/drinks; xerostomia; more than one interproximal lesion; special health care needs; low socioeconomic status
Document clinical indication: number of surfaces involved, extent of lesion (cavitated vs non‑cavitated), radiographic findings, and rationale for crown vs alternative restoration

Denial Risk

Potential denial triggers

Use of prefabricated crowns solely for cosmetic purposes or as a preventive measure on teeth with no evidence of pathology may be denied. Other denial triggers include placing a crown on a primary tooth that is close to exfoliation (more than half root resorption), cases with excessive crown loss preventing mechanical retention, or when loss of space/tipping prevents proper fit.

Prefabricated crowns are full-coverage restorations commonly used to restore primary teeth and protect tooth structure in high caries risk patients. Typical indications include restoring teeth with extensive carious lesions, following pulpal therapy, managing developmental defects, serving as abutments for space maintainers, and protecting teeth with extensive surface loss or where isolation for intracoronal restorations is not possible. They are generally selected for best fit, adapted, and cemented with a biocompatible luting agent and may be made of stainless steel, porcelain/ceramic, or acrylic.
