State-specific note: This policy applies only to New Jersey (New Jersey Medicaid/NJ FamilyCare) and does not address treatment for obstructive sleep apnea or temporomandibular joint disorders; refer to separate New Jersey policies for those conditions.

Effective date: February 1, 2026.

Conditional coverage requirements: Coverage requires documentation of verifiable objective clinical measurements (e.g., overjet, molar discrepancy, open bite measurements, asymmetry in mm per policy thresholds) AND evidence of functional impairment (masticatory/swallowing dysfunction or speech impairment) to support medical necessity.

Medical-Necessity Criteria

Medically Necessary / Reconstructive Indications

Orthognathic (jaw) surgery may be considered Reconstructive and medically necessary when ALL of the following are met:

ALL of the following

Anteroposterior discrepancies (established norm = 2 mm) — ONE of
- Maxillary/mandibular incisor relationship: Horizontal overjet of 5 mm or more or a 0 to a negative valueHorizontal overjet >= 5 mm or 0 to negative
- Maxillary/mandibular anteroposterior molar relationship: Discrepancy of 4 mm or more (norm 0 to 1 mm)Molar AP relationship discrepancy >= 4 mm
Vertical discrepancies — ONE of
- Presence of a vertical facial skeletal deformity which is two or more standard deviations from published norms for accepted skeletal landmarks
- Open bite — ONE of
  - No vertical overlap of anterior teeth
  - Unilateral or bilateral posterior open bite greater than 2 mmPosterior open bite > 2 mm
- Deep overbite with impingement or irritation of buccal or lingual soft tissues of the opposing arch
- Supraeruption of a dentoalveolar segment due to lack of occlusion
Transverse discrepancies — ONE of
- Presence of a transverse skeletal discrepancy which is two or more standard deviations from published norms
- Total bilateral maxillary palatal cusp to mandibular fossa discrepancy of 4 mm or greater, or a unilateral discrepancy of 3 mm or greater, given normal axial inclination of the posterior teethBilateral >= 4 mm or unilateral >= 3 mm

Prior authorization for cosmetic claims with medical necessity

If a procedure would otherwise be considered cosmetic but significant redeeming medical necessity can be demonstrated, the physician must submit a written prior authorization request to the Medical Assistance Customer Center. Requests shall be submitted in writing and shall include photographs when indicated to support the request. The physician shall obtain prior authorization from the Medical Assistance Customer Center before this service is rendered.

Providers must document verifiable clinical measurements (for example: horizontal overjet, molar anteroposterior discrepancy, open bite measurements, and asymmetry in millimeters) and provide evidence of functional impairment such as masticatory or swallowing dysfunction (e.g., inability to bite/chew solid foods, choking on incompletely masticated foods, damage to soft tissue during mastication, malnutrition) or documented speech impairment to support medical necessity.

Background, Evidence & Definitions

Background: Orthognathic surgery corrects skeletal abnormalities of the mandible (lower jaw), maxilla (upper jaw), or both to improve facial form and physiologic function. These abnormalities may be congenital, result from traumatic injuries, or be secondary to systemic disease, and treatment often is combined with orthodontics and other rehabilitative therapies.

Purpose and goals: The primary goal of orthognathic surgery is to restore or improve physiologic function (for example, mastication, swallowing, and speech) rather than to provide cosmetic improvement; correction is performed to address skeletal deformities associated with masticatory malocclusion and related functional impairments.
