Coverage Criteria for Orthognathic Surgery

inv-02: Functional impairment requirement

In addition to measurable skeletal deformity, one or more of the following functional impairments must be documented:

Masticatory/Swallowing dysfunction: Masticatory and swallowing dysfunction due to skeletal malocclusion (e.g., inability to bite and/or chew solid foods, choking on incompletely masticated solid foods, damage to soft tissue during mastication, malnutrition)clinical

Speech impairment: Documentation of speech impairment due to facial skeletal deformityclinical

This policy excludes procedures performed solely for cosmetic purposes and any procedures that correct an anatomical congenital anomaly without improving or restoring physiologic function. The policy defines cosmetic procedures as those that change or improve appearance without significantly improving physiological function, and specifies that correction of a congenital anomaly is considered cosmetic if it does not restore physiologic function. Such cases do not meet the policy’s requirements for reconstructive or medically necessary orthognathic surgery and may be denied.

This policy does not address surgical treatment for obstructive sleep apnea or temporomandibular joint (TMJ) disorders. For those conditions, refer to the Nebraska-specific Medical Policies titled “Obstructive and Central Sleep Apnea Treatment (for Nebraska Only)” and “Treatment of Temporomandibular Joint Disorders (for Nebraska Only).”

For informational purposes only: orthognathic surgery is a surgical procedure and is not subject to regulation by the U.S. Food and Drug Administration. FDA approval alone is not a basis for coverage decisions under this policy.

Orthognathic surgery performed solely to change or improve appearance is not considered reconstructive or medically necessary. When the primary purpose of the procedure is cosmetic or to correct appearance without restoring physiologic function, it does not satisfy the policy’s reconstructive criteria and may be denied.

Indications include congenital, developmental, traumatic, or disease‑related skeletal discrepancies that are associated with masticatory malocclusion. The policy aligns with professional guidance and requires measurable skeletal deformities (for example, horizontal overjet ≥ 5 mm, molar discrepancy ≥ 4 mm, asymmetry > 3 mm, transverse cusp‑to‑fossa discrepancy bilateral ≥ 4 mm or unilateral ≥ 3 mm, or posterior open bite > 2 mm) and documentation of one or more associated functional impairments such as masticatory/swallowing dysfunction or documented speech impairment. Documentation Required

Documentation Requirements — Demonstrate Specific Skeletal Measurements and Functional Impairment

Documentation submitted for prior authorization and coverage determinations must demonstrate the specific skeletal deformity measurements and associated functional impairments that meet medical necessity criteria.

Include objective clinical measurements (e.g., horizontal overjet in mm, molar relationship discrepancy, vertical open bite measurements, transverse discrepancies, asymmetry measurements).
Provide documentation of functional impairment: masticatory/swallowing dysfunction, speech impairment, airway dysfunction when relevant, or other physiologic impairments.
Attach pre- and post-treatment orthodontic plans, imaging (cephalometric analyses, dental models, photographs), and clinical notes supporting severity and functional impact.

Multidisciplinary Treatment Expectations

Orthognathic care is expected to be multidisciplinary. Authorization and documentation should reflect comprehensive team-based treatment planning.

Provide documentation of pre-surgical and planned post-surgical orthodontic treatment when applicable.
Include evidence of multidisciplinary coordination (surgeon, orthodontist, speech therapy, ENT or sleep specialists when airway issues are present).
Ensure treatment plans show timing and roles of each team member (e.g., pre-surgical orthodontic alignment, surgical plan, post-surgical orthodontic finishing).
