Overview: Orthognathic surgery is considered Reconstructive and medically necessary when the InterQual CP criteria for orthognathic surgery are met or when documented, measurable skeletal deformity criteria AND at least one qualifying functional impairment are present. Measurable skeletal criteria include specific numeric thresholds for anteroposterior, vertical, transverse, and asymmetry deformities (e.g., horizontal overjet ≥ 5 mm or 0 to negative value; molar discrepancy ≥ 4 mm; vertical deformity ≥ 2 standard deviations from norms; posterior open bite > 2 mm; transverse discrepancy ≥ 2 standard deviations; bilateral palatal cusp to mandibular fossa discrepancy ≥ 4 mm or unilateral ≥ 3 mm; asymmetry > 3 mm with occlusal asymmetry). Required functional impairments include masticatory/swallowing dysfunction (examples: inability to bite/chew solid foods, choking on incompletely masticated foods, tissue damage during mastication, malnutrition) or documented speech impairment. Background: Orthognathic surgery corrects skeletal abnormalities of the mandible and/or maxilla to improve facial form and physiologic function. These abnormalities may be congenital, developmental, traumatic, or disease-related. The primary goal is to restore facial form and function, and treatment is often performed in combination with adjunctive orthodontic therapy (pre- and/or postoperative orthodontics) to optimize occlusion and long-term outcomes.

Systematic reviews/meta-analyses: Multiple systematic reviews (2017–2023) report that orthognathic surgery improves masticatory function and yields variable long-term stability; overall evidence is limited by retrospective study designs, small sample sizes, and heterogeneous follow-up.
