Coverage and Medical Necessity Criteria

Reconstructive / Medically Necessary Criteria

Orthognathic surgery is considered reconstructive and medically necessary when ALL of the following are met:

Primary anatomical criteria

Anteroposterior discrepancies: Maxillary/mandibular incisor relationship: horizontal overjet >= 5 mm or zero to negative OR maxillary/mandibular anteroposterior molar relationship discrepancy >= 4 mm (norm 0–1 mm)incisor overjet >=5 mm or 0/negative; molar AP discrepancy >=4 mm
Vertical discrepancies: Vertical facial skeletal deformity >=2 standard deviations from published norms OR open bite (no anterior vertical overlap OR unilateral/bilateral posterior open bite > 2 mm) OR deep overbite with soft-tissue impingement OR supraeruption of a dentoalveolar segment>=2 SD or posterior open bite >2 mm
Transverse discrepancies: Transverse skeletal discrepancy >=2 standard deviations from published norms OR total bilateral maxillary palatal cusp to mandibular fossa discrepancy >= 4 mm OR unilateral discrepancy >= 3 mm (with normal axial inclination)>=2 SD or bilateral >=4 mm or unilateral >=3 mm
Asymmetries: Anteroposterior, transverse, or lateral asymmetries > 3 mm with concomitant occlusal asymmetry>3 mm

Functional impairment requirement: Individual must have one or more of: masticatory and swallowing dysfunction due to skeletal malocclusion (examples: inability to bite/chew solid foods, choking on incompletely masticated foods, soft tissue injury during mastication, malnutrition) OR documented speech impairment due to facial skeletal deformityN/A

Medical necessity criteria

Covered when ALL of the following are met

Primary clinical criteria: Presence of a facial skeletal abnormality with associated functional medical impairment (e.g., masticatory dysfunction, airway dysfunction, TMJ disorder, speech disorder, or psychosocial disorder) documented and measured.

See AAOMS measurable thresholds for specific anteroposterior, vertical, transverse discrepancies and asymmetries.

Objective measurements: Clinical measurements meet AAOMS thresholds (examples: horizontal overjet >= +5 mm or zero/negative; molar AP discrepancy >= 4 mm; transverse discrepancy bilateral palatal cusp to mandibular fossa >= 4 mm; unilateral >= 3 mm; asymmetries > 3 mm; vertical deformity >=2 SD from norms; open bite criteria).AAOMS numeric thresholds

Measurements should be verifiable on diagnostic imaging and cephalometric analysis.

Documentation: Treating physician's plan of care with surgical objectives and reports of recent, high-quality diagnostic imaging (cephalometric tracings/analyses and radiographs) demonstrating the abnormality and its contribution to impairment.

This policy does not address surgical treatment for obstructive sleep apnea or temporomandibular joint (TMJ) disorders. Providers should refer to the separate Medical Policies titled Obstructive and Central Sleep Apnea Treatment or Treatment of Temporomandibular Joint Disorders for coverage guidance when those conditions are the primary indication for surgery. Additionally, orthognathic procedures performed in the context of orthognathic–orthodontic care for jaw realignment are covered only when the documented anatomic and functional criteria in this policy are met; procedures performed solely for different surgical indications should be evaluated under the applicable policy.

For informational purposes only: orthognathic surgery is a surgical procedure and is not subject to U.S. Food and Drug Administration (FDA) regulation. As such, FDA approval or clearance status is not, by itself, a basis for coverage decisions under this policy.

Orthognathic surgery is not considered reconstructive and medically necessary when the indication is cosmetic only or when the documented facial skeletal abnormality and associated functional impairment do not meet the specific anatomic and numeric thresholds set forth in this policy. Requests primarily for aesthetic improvement without verified masticatory, swallowing, speech, airway, TMJ, or psychosocial functional impairment are subject to denial.

Prior authorization is required for orthognathic (jaw) surgical procedures. Submit a prior authorization request that documents the presence of a facial skeletal deformity meeting the numeric thresholds in policy (e.g., anteroposterior, vertical, transverse discrepancies, or asymmetries) and associated functional impairment. Requests submitted without required documentation may be delayed or denied.

Documentation must demonstrate the facial skeletal deformity using numeric measurements meeting the policy thresholds (for example: horizontal overjet ≥ 5 mm, molar AP discrepancy ≥ 4 mm, asymmetry > 3 mm, transverse palatal cusp–fossa discrepancy ≥ 4 mm bilateral or ≥ 3 mm unilateral, open bite or vertical deformity ≥ 2 SD from norms). Also include objective evidence of functional impairment (masticatory/swallowing dysfunction, documented speech impairment, tissue impingement, or malnutrition).

Background and Evidence Summary

Orthognathic surgery involves surgical correction of skeletal abnormalities of the mandible (lower jaw) and/or maxilla (upper jaw). These abnormalities may be congenital, developmental, traumatic, or disease-related, and are treated to improve facial form and physiologic function. Include the treating surgeon's plan of care describing surgical treatment objectives and the expected outcome for improvement of the functional deficit. Provide recent high-quality diagnostic imaging and radiologic interpretations addressing the abnormality and the degree to which it causes impairment.

Role of Non‑surgical/Orthodontic Therapy

Non-surgical and orthodontic therapies are commonly part of management. Orthodontic treatment alone may improve occlusion and masticatory function; document prior or concurrent non-surgical management and the clinical rationale for proceeding with surgery.
