Because of these limitations, the definitive balance of risks and benefits and which patients are most likely to benefit remain uncertain.

Corticotomy (also referenced as periodontally accelerated osteogenic orthodontics/PAOO or surgically assisted osteogenic orthodontics/SAOO) is characterized in this policy as unproven due to insufficient high‑quality evidence demonstrating sustained efficacy and safety. While multiple studies suggest temporary acceleration of tooth movement, the overall body of evidence lacks well‑designed, larger trials with long‑term outcomes to confirm durable clinical benefit.

Given the limitations of the evidence base, corticotomy/PAOO procedures cannot be universally recommended as medically necessary for all patients. The current literature does not provide consistent, high‑quality data to support a blanket coverage position; decisions regarding use should therefore be individualized and informed by the treating clinician’s judgment and the patient’s specific circumstances.
