Coverage Criteria for Oral Surgery Excisional Procedures

Covered indications (by procedure)

Covered when ANY of the procedure-specific indications below are met:

Frenulectomy/Frenuloplasty indications: When attachment of the frenum is coronal to the mucogingival junction, within the free gingiva, or in the papilla causing a diastema, gingival recession, or stripping; when the position/attachment of the frenum interferes with proper oral hygiene; prior to construction of a removable denture replacing teeth in the area of aberrant frenal attachment; when there is a functional disturbance (including but not limited to mastication, swallowing, and speech); for ankyloglossia or papillary penetrating attachment of maxillary labial frenum in newborns when there is interference with feeding.

Excision of Hyperplastic Tissue / Surgical Reduction of Fibrous Tuberosity: When excess tissue interferes with the fit of a partial or complete denture (existing or new).

Excision of Pericoronal Gingiva: For recurrent infections of the operculum around impacted or partially erupted lower third molars; when an erupted maxillary third molar is traumatizing soft tissue around an opposing tooth; when the presence interferes with the fit of a partial or complete denture.

Transseptal Fiberotomy / Supra crestal Fiberotomy (by report): To reduce rotational relapse of individual teeth following orthodontic treatment.

Removal of Lateral Exostosis / Torus Palatinus / Torus Mandibularis: If a partial or complete denture cannot be adapted successfully; when causing soft tissue trauma with existing removable appliances; for unusually large protuberances prone to recurrent traumatic injury; when causing a functional disturbance (including but not limited to mastication, swallowing, and speech).

Excisional oral surgery procedures may not be indicated for individuals with certain medical or pharmacologic risk factors that can impair healing. Specifically, procedures may be contraindicated for people with an unmanaged medical condition (examples include but are not limited to metabolic, cardiovascular, autoimmune/inflammatory disorders, and genetic conditions affecting collagen synthesis) and for individuals taking medications that negatively affect the healing response (examples include immunosuppressive agents, corticosteroids, anticoagulants, NSAIDs, and nicotine).

The policy explicitly identifies clinical situations in which excisional procedures may be considered not indicated (see exclusions) and therefore could be not medically necessary. In addition to the unmanaged medical conditions and healing‑impairing medications noted above, removal of lateral exostoses, torus palatinus, and torus mandibularis is also potentially not indicated for individuals who have undergone radiation therapy to the head and neck.

Common indications include management of problematic frenum attachments (for diastema, gingival recession, interference with hygiene, feeding, or prosthesis construction), excision of excess soft tissue that interferes with denture fit, treatment of recurrent pericoronal infections, and procedures such as transseptal fiberotomy to reduce orthodontic rotational relapse.
