Clinical Background, Evidence, and Definitions

Background: Mucogingival conditions are deviations from the normal anatomic relationship between the gingival margin and the mucogingival junction. Surgical grafting procedures are described as the gold standard to correct localized gingival defects and provide a functionally adequate zone of attached gingiva.

Factors affecting success: Success of these procedures is highly dependent on individual patient considerations such as level of oral hygiene, smoking status, and overall health. The policy also notes situations where procedures may not be indicated (for example unmanaged medical conditions, medications that impair healing) and specific donor‑site limitations for autogenous grafts.

Evidence — Cochrane review (2018): The Cochrane systematic review evaluated 48 randomized controlled trials and found that subepithelial connective tissue grafts plus coronally advanced flap showed greater reduction in gingival recession versus guided tissue regeneration with resorbable membranes plus CAF; overall risk of bias and evidence quality were variable.

Evidence — Zucchelli et al. (2014): A randomized trial of CAF versus CAF + CTG for multiple adjacent recessions showed no significant difference at 6 months and 1 year, but at 5 years the CAF + CTG group demonstrated statistically greater recession reduction, higher probability of complete root coverage, greater increase in buccal keratinized tissue height, and better contour evaluation.

Evidence — Kuis et al. (2013): A 5‑year split‑mouth randomized trial comparing CAF alone versus CAF + CTG for single defects found both procedures effective, but CAF + CTG provided significantly better results across evaluated clinical parameters and better long‑term outcomes (60 months) than CAF alone.
