Medical-Necessity Criteria

Nitrous Oxide Indications and Contraindications

Nitrous Oxide may be indicated when any of the following are present and may be contraindicated/absolutely contraindicated as listed:

ANY of the following

Extensive and/or complex procedures (e.g., serial extractions, periodontal surgeries)
Individuals with physical, cognitive, or developmental disabilities
Ineffective Local Anesthesia
Management of a severe gag reflex
Management of fear and anxiety

May be contraindicated for

Claustrophobia
Pregnancy
Severe underlying medical conditions (e.g., critical illnesses, cardiac disease, pulmonary hypertension)
Significant personality and behavioral disorders
Upper respiratory tract infections or other respiratory conditions

Intravenous Moderate/Conscious Sedation and Deep Sedation/General Anesthesia Indications

Intravenous moderate/conscious sedation and deep/general anesthesia may be indicated when any of the following are present:

ANY of the following

Allergy or sensitivity to local anesthetic agents
Extensive and/or complex procedures (e.g., serial extractions, periodontal surgeries)
Anxiety and fear when other techniques have proven inadequate
Behavioral management when other techniques have proven inadequate
Individuals that are medically compromised or those with special needs

Non-Intravenous Sedation Indications

Non-intravenous sedation may be indicated when any of the following are present:

ANY of the following

Individuals with physical, cognitive, or developmental disabilities
Mild to moderate apprehension and anxiety

Definitions

Deep Sedation: A drug-induced depression of consciousness during which patients cannot be easily aroused but respond purposefully following repeated or painful stimulation; ventilatory function may be impaired and patients may require an advanced airway; spontaneous ventilation may be inadequate; cardiovascular function is usually maintained.

General Anesthesia: A drug-induced loss of consciousness during which patients are not arousable, even by painful stimulation; ventilatory function is often impaired and patients often require an advanced airway and may require positive pressure ventilation; cardiovascular function may be impaired.

Local Anesthesia: Elimination of sensation, especially pain, in one part of the body by topical application or regional injection of a drug.

Moderate Sedation ('Conscious Sedation'): A drug-induced depression of consciousness during which patients respond purposefully to verbal commands, alone or with light tactile stimulation; no interventions are required to maintain a patent airway and spontaneous ventilation is adequate; cardiovascular function is usually maintained.

Nitrous Oxide: A colorless, odorless to sweet-smelling inorganic gas that when combined with oxygen can be a safe and effective means of managing pain and anxiety.

Non-Intravenous Sedation: Sedation medications delivered through the oral, intranasal, or transmucosal routes.

This policy aligns with guidance from the ADA, ASA, and AAPD and requires that dentists comply with applicable state laws, training, and educational requirements for the intended level of sedation; ADA guidance includes patient history, personnel/equipment requirements, monitoring (consciousness, oxygenation, ventilation, circulation), recovery/discharge, and emergency management.
