General coverage stance for adjunctive diagnostic tests

Summary stance based on evidence and guidelines:

Adjunctive tests cannot replace biopsy: Adjunctive tests (vital staining, light-based devices, autofluorescence, salivary/blood biomarkers) do not have sufficient diagnostic accuracy to replace surgical biopsy and histologic assessment; oral cytology demonstrates the most potential but remains adjunctive

Cochrane review and ADA and NCCN guidance support biopsy as the diagnostic gold standard and limit routine use of adjunctive tests.

Use of cytologic adjuncts when biopsy not immediately feasible

When biopsy is declined or access limited:

Cytology as temporary adjunct: If a patient declines biopsy or lacks access to timely biopsy, cytologic adjuncts (brush cytology) may be used to initiate the diagnostic process until a biopsy can be performed; positive or atypical cytology should prompt biopsy or specialist referral; negative cytology requires periodic follow-up and biopsy if the lesion persists or progresses

ADA guideline allows cytologic adjuncts only as an interim measure when biopsy is declined or not available.

Antigen and antibody testing, and molecular testing for public health–related pathogens (for example, testing for coronavirus) are out of scope when performed by dental providers within the dental office. Providers should refer to the Description of Services and applicable public health guidance for appropriate venues and procedures for these tests.

Adjunctive tests — including vital staining, light-based devices, oral spectroscopy, salivary or blood biomarker assays, and commercially available salivary adjuncts — are not recommended as replacements for surgical biopsy and histologic assessment. Evidence reviews and guideline statements show these tools lack sufficient diagnostic accuracy to serve as standalone diagnostic tests; oral cytology demonstrates the most promise but remains an adjunct that should prompt biopsy or specialist referral when results are positive or atypical.

These technologies may aid clinical evaluation but cannot replace biopsy and histopathology when malignancy or a potentially malignant disorder is suspected.

Biopsy or Referral Expectation

When oral mucosal lesions are suspicious, the ADA guideline recommends performing a biopsy or referring to a specialist for definitive histopathologic diagnosis. A biopsy or specialist referral is the expected clinical action when indicated.

If a lesion is clinically suspicious, perform a scalpel biopsy or refer to an appropriate specialist for biopsy and histopathologic evaluation.
