Guidelines for dental implant placement and patient selection

Placement criteria: Implant may be placed at time of extraction; implant site must be free of infection; implant must be a minimum of 1.5-2 mm from adjacent tooth roots.

Individual factors to consider: Patient ability/willingness for oral hygiene and routine care; occlusal load; bone quality/quantity; history of or active periodontal disease; adequate restorative space; patient age; presence of conditions/treatments that may interfere with normal healing (examples: chemotherapy, radiation to head/neck, uncontrolled diabetes or hypertension, recent myocardial infarction or stroke, anticoagulant therapy, blood dyscrasias, IV bisphosphonate therapy, estrogen deficiency); significant psychiatric disorder or intellectual disability; lifestyle risks such as smoking, drug addiction, alcoholism.

Treatment of peri-implant defects/disease

Treatment options: Non-surgical periodontal therapy; surgical treatment including flap and closure, which may include osseous contouring; bone grafting and guided tissue regeneration procedures as described in applicable codes.

Implant mobility is a contraindication to treatment for peri-implant defects/disease.

Treatment of peri-implant defects or disease is not indicated if an implant is mobile. Mobile implants indicate loss of implant stability and osseointegration, and interventions aimed at treating peri-implant mucositis or peri-implantitis are unlikely to be effective when the implant itself is not stably integrated into bone.

Placement of an implant is contraindicated when the implant site has an active infection or when individual patient factors contraindicate normal healing. Examples of such patient factors include recent or ongoing systemic conditions and therapies that impair wound healing (for example, uncontrolled diabetes, recent chemotherapy or head/neck radiation, blood dyscrasias, or IV bisphosphonate therapy), significant uncontrolled cardiovascular events, ongoing anticoagulant therapy, poor oral hygiene or uncontrolled periodontal disease, and lifestyle risks such as smoking or substance abuse. These conditions may render implant placement inappropriate until they are addressed and the site is free of infection.

Step Therapy

Stepwise Treatment Approach

Non-surgical periodontal therapy and appropriate sequencing of care are required prior to certain surgical interventions for peri-implant disease. Treat peri-implant defects stepwise: begin with non-surgical periodontal therapy; if inadequate, escalate to surgical interventions (flap and closure, osseous contouring as indicated). Surgical treatment is not indicated if the implant is mobile.
