Coverage Criteria for Restorative Foundations, Posts/Cores, and Pins

Medically Necessary Indications

Covered when ALL of the following clinical conditions are met:

General: Tooth has insufficient remaining structure to retain the planned restoration and the procedure is intended to provide a restorative foundation or retention for an indirect or direct restoration.

Restorative Foundation for an Indirect Restoration: Restorative foundation is indicated to eliminate undercuts, voids, or other preparation irregularities to create a favorable tooth form for retention of an indirect restoration.

Core Buildup Indication: Core buildup (including any pins when required) is indicated for teeth with significant loss of coronal tooth structure due to caries or trauma where insufficient tooth structure remains to adequately retain an indirect restoration.

Post and Core Indication: Post and core is indicated for endodontically treated teeth with significant loss of coronal tooth structure when insufficient tooth structure remains to retain an indirect restoration, or when inadequate remaining tooth structure exists to support a core.

Post and Core -- Contraindications: Post and core are not indicated for teeth with short roots; a post is not indicated when anatomic features (e.g., canals and pulp chamber) can retain a core.

Pin Retention Indication: Pin retention is indicated for teeth with significant loss of coronal tooth structure to allow retention of a direct restoration.

Post Removal Indications: Post removal is indicated for loss of adequate retention; fracture of the tooth and/or post and core; recurrent caries associated with post and core; need for access to the root canal system for non‑surgical endodontics; or when the tooth has a reasonable long‑term prognosis for a new restoration.

Core buildup is not indicated in the following situations: as a filler to correct irregularities in a tooth preparation; as a definitive composite or amalgam restoration in place of a final restoration; or solely for retention of intracoronal restorations. Claims for core buildup used for these purposes may be denied.

A post is not indicated when anatomic features of the tooth can retain the core, such as when the canals and pulp chamber provide sufficient retention for the restorative foundation. Additionally, post and core placement is not indicated for teeth with short roots. Providers should document when anatomic retention is insufficient before placing a post.

Documentation Required

Clinical justification

Clinical documentation must support the need for a core buildup, post and core, or pin retention. Record findings that demonstrate insufficient remaining tooth structure to retain an indirect restoration, rationale for chosen retention method, and overall favorable long-term prognosis for the tooth.

Document clinical findings showing inadequate coronal tooth structure
Include radiographs and charting that support endodontic status and restorative plan
Document rationale for selecting post, core, or pin and why alternative options are unsuitable

Documentation Required

Post removal justification

When removing an existing post, document the specific indication(s) for removal such as loss of retention, fracture of the tooth or post and core, recurrent caries involving the post and core, need for access to the root canal system for non-surgical endodontic retreatment, and evidence that the tooth has a reasonable long-term prognosis for a new restoration.

Include clinical notes and radiographs demonstrating the indicated reason(s) for post removal
If removal is for endodontic access, document planned endodontic procedure

Denial Risk

Improper use of core buildup and inappropriate post/pin placement

Core buildup, posts, and pins have specific appropriate and inappropriate uses. Do not use core buildup merely to correct preparation irregularities, as a definitive composite or amalgam restoration, or to retain intracoronal restorations without clinical justification. Posts are indicated for endodontically treated teeth with significant coronal tooth structure loss when insufficient structure remains to retain an indirect restoration, and when roots are of adequate length. Posts are not indicated for teeth with short roots or when anatomic features can adequately retain the core. Pin retention is not indicated for teeth with significant malocclusion or when the tooth cannot be properly restored due to anatomic or functional considerations.
