Document the member's age (member must be under age 19 — through age 18 — unless the member specific benefit plan document indicates a different age). Also document clinical evidence that services are related to a severe craniofacial deformity resulting in a Handicapping Malocclusion (examples include, but are not limited to, cleft lip/palate, Crouzon syndrome, hemifacial hypertrophy, Parry-Romberg syndrome, Pierre‑Robin sequence, and Treacher‑Collins syndrome). The age requirement is per this policy.

Background

Background: This policy addresses orthodontic treatment that is medically necessary to correct the dental component of craniofacial abnormalities causing a Handicapping Malocclusion and to restore a functional dentition.

What is excluded: Routine or cosmetic orthodontics are not covered under this policy, including treatment for crowded dentitions (crooked teeth), excessive spacing, temporomandibular joint (TMJ) conditions, or horizontal/vertical discrepancies (overjet/overbite) when there is no qualifying craniofacial deformity.

Purpose: The policy is intended to ensure coverage for orthodontic services when they are necessary to manage the dental component of craniofacial deformities that produce a physically handicapping malocclusion; for Essential Health Benefits (EHB) criteria submission forms by state, refer to the Health Care Criteria for Medically Necessary Orthodontia.

Definitions:

Handicapping Malocclusion: A physical condition that interferes with normal functioning due to deviation in intra/intermaxillary relations of teeth often associated with dentofacial deformities (AAO).

Cleft Lip / Cleft Palate: Congenital facial defects due to failure of fusion of facial prominences (American Cleft Palate-Craniofacial Association).

Crouzon Syndrome/Craniofacial Dysostosis: Facial birth defects with abnormal craniofacial fusion affecting bone growth and facial/teeth relationships (American Cleft Palate-Craniofacial Association).

Hemifacial Hypertrophy/Congenital Hemifacial Hyperplasia: Rare developmental anomaly with asymmetric overgrowth; may be isolated or syndromic (Neville 2016).

Parry-Romberg Syndrome/Progressive Hemifacial Atrophy: Rare disorder with progressive deterioration of the skin and soft tissues of half the face (NIH).

Pierre-Robin Syndrome: Complex of congenital anomalies including micrognathia and often cleft palate (American Cleft Palate-Craniofacial Association).

Treacher-Collins Syndrome/Mandibulofacial Dysostosis: Birth defect affecting the size and shape of facial structures with variable deformity (American Cleft Palate-Craniofacial Association).
