Scope: This policy defines indications, contraindications, and related guidance for fixed partial dentures (fixed bridges), resin-bonded appliances, provisional fixed partial dentures, repairs, precision attachments, connector bars, and stress breakers, and lists applicable CDT procedure codes. Coverage is subject to the member-specific benefit plan and any applicable federal/state mandates; the member-specific benefit plan governs in case of conflict.

Medical Necessity Criteria

Indications for Fixed Partial Dentures

Fixed partial dentures may be indicated when ALL of the following are met:

ALL of the following

Replacement of missing permanent teeth in which the Retainer/Abutment teeth have a favorable long-term prognosis
Replacement of one to two missing teeth in a Tooth Bounded Space

Indications for Resin Bonded Appliances (e.g., Maryland Bridge)

Resin bonded appliances are indicated when ALL of the following are met:

ALL of the following

Replacement of one missing tooth
Retainer/Abutment teeth are unrestored/undamaged

Contraindications / Not Indicated

Fixed partial dentures are not indicated when ANY of the following are present:

ANY of the following

Members with rampant caries and/or poor oral hygiene
Retainer/Abutment teeth have untreated endodontic pathology or periodontal disease or an unfavorable crown root ratio
Teeth intended as Retainers/Abutments have inadequate remaining tooth structure
A tooth to be used as a Retainer/Abutment has tipped or drifted into edentulous space

Indications for Provisional Fixed Partial Dentures

Provisional fixed partial dentures may be indicated when ANY of the following apply:

ANY of the following

A permanent fixed partial denture does not have a favorable long-term prognosis
To replace a lost tooth in members with a Mixed Dentition to allow maturity of the dentition and jaws before constructing a definitive fixed prosthetic appliance
When a systemic medical condition prohibits the placement of a definitive fixed prosthetic appliance

Fixed Partial Denture Repair (Necessitated by Restorative Material Failure)

Repair may be indicated when ALL of the following are met:

The appliance to be repaired is functional
The appliance has a favorable long-term prognosis

Indications for Precision Attachments

Precision attachments may be indicated when ANY of the following apply:

When aesthetics need to be considered

Cantilever Fixed Dental Prosthesis	A fixed complete or partial denture in which the Pontic is retained and supported only on one end by one or more Abutments.
Connector Bar	Device attached to fixed partial denture Retainer or coping to stabilize and anchor a removable overdenture prosthesis.
Fixed Dental Prosthesis	Any prosthesis securely fixed to natural teeth or implants that cannot be removed by the patient.
Mixed Dentition	Approximately age 6 to 13 when primary and permanent teeth are present.
Pontic	An artificial tooth on a fixed dental prosthesis that replaces a missing natural tooth.
Precision Attachment	An interlocking device with one component fixed to an Abutment and the other integrated into a removable dental prosthesis.
Resin-Bonded Prosthesis	A fixed prosthesis luted to tooth structures, primarily enamel, etched to provide mechanical retention for resin cement.
Retainers	Any device used for the stabilization or retention of prosthesis.
Stress Breaker	Part of a prosthesis designed to relieve Abutment teeth and supporting tissues from harmful stresses.
Tooth Bounded Space	A space created by one or more missing teeth that has a tooth on each side.
