Denial Risk

Certain services related to weight management are not covered and may be denied, including behavioral modification programs, dietary reviews and counseling (except as provided by the primary care physician), exercise programs, many supplements, vitamins, and other non‑prescription weight-related products. Prescription obesity drugs are not covered under this benefit unless covered under the supplemental pharmacy benefit, authorized as medically necessary to treat morbid obesity, or otherwise listed as covered in the Covered Benefits section.
