Reauthorization — North Dakota EHB (Wegovy or Zepbound)

Reauthorization — North Dakota Fully Insured EHB (Wegovy or Zepbound): Applies to small group and individual fully insured plans in North Dakota.

ALL of the following

Reauthorization — Vykat XR: Continued coverage requires evidence of ongoing symptomatic benefit for hyperphagia associated with Prader-Willi Syndrome.

ALL of the following

Documentation of positive clinical response to Vykat XR therapy, including reduction in hyperphagia behaviors or other validated symptom measures.
Prader-Willi Syndrome diagnosis remains confirmed by genetic testing.
Patient continues to meet the age requirement (at least 4 years of age).
Authorization will be issued for 12 months for continuation when reauthorization criteria are met.
