Medical-Necessity Criteria

Nitrous Oxide - Indications and Contraindications

Nitrous Oxide may be indicated when ANY of the following are present; contraindications list conditions where use is inadvisable or absolutely contraindicated.

ANY of the following

Extensive and/or complex procedures (e.g., serial extractions, periodontal surgeries)
Individuals with physical, cognitive, or developmental disabilities
Ineffective Local Anesthesia
Management of a severe gag reflex
Management of fear and anxiety

Relative Contraindications (may be contraindicated)

Claustrophobia
Pregnancy
Severe underlying medical conditions (e.g., critical illnesses, cardiac disease, pulmonary hypertension)
Significant personality and behavioral disorders
Upper respiratory tract infections or other respiratory conditions

Absolute Contraindications

Methylenetetrahydrofolate reductase (MTHFR) deficiency
Pulmonary hypertension
Severe chronic obstructive pulmonary disease (COPD)
Treatment with bleomycin sulfate
Vitamin B12 deficiency
Within three months of vitreoretinal surgery

Intravenous Moderate/Conscious Sedation and Deep Sedation/General Anesthesia - Indications and Considerations

IV moderate/conscious sedation and deep sedation/general anesthesia may be indicated when ANY of the following are met; contraindications and risk factors noted elsewhere may preclude use.

ANY of the following

Allergy or sensitivity to local anesthetic agents
Extensive and/or complex procedures (e.g., serial extractions, periodontal surgeries)
Anxiety and fear when other techniques have proven inadequate
Behavioral management when other techniques have proven inadequate
Individuals that are medically compromised or those with special needs

Non-Intravenous Conscious Sedation - Indications

Non-intravenous sedation may be indicated when ANY of the following are present:

Mild to moderate apprehension and anxiety

Definitions

Deep SedationA drug-induced depression of consciousness during which patients cannot be easily aroused but respond purposefully following repeated or painful stimulation; ventilatory function may be impaired; patients may require airway assistance.

General AnesthesiaA drug-induced loss of consciousness during which patients are not arousable, even by painful stimulation; ventilatory function often impaired and positive pressure ventilation may be required.

Local AnesthesiaElimination of sensation in one part of the body by topical application or regional injection of a drug.

Moderate SedationA drug-induced depression of consciousness during which patients respond purposefully to verbal commands; no interventions are required to maintain airway; spontaneous ventilation is adequate.

Nitrous OxideA colorless, odorless to sweet-smelling inorganic gas which, when combined with oxygen, can be a safe and effective means of managing pain and anxiety.

Non-Intravenous SedationSedation medications delivered through oral, intranasal, or transmucosal routes.
