When a member changes carriers during an inpatient stay, the provider must determine whether the prior or succeeding carrier is responsible for the remainder of the inpatient stay and take appropriate billing and authorization actions. Verify the member’s coverage dates, product type (including whether the plan is an HMO, New York insurance product, or self-funded plan), and whether the member elected coverage under the succeeding carrier. If responsibility is unclear, obtain written confirmation from the carrier identified as responsible before billing or seeking payment.

Confirm member’s coverage effective and termination dates with both prior and succeeding carriers.
Determine whether the member’s product is a New York insurance product, an HMO, or a self-funded plan (extended benefits provisions differ).
Document whether the member elected coverage under the succeeding carrier — member election can shift responsibility to the succeeding carrier.
If Oxford is prior carrier and coverage terminated while member was inpatient, Oxford generally remains responsible for remainder of stay unless exceptions above apply.
If Oxford is succeeding carrier and coverage began while member was inpatient, the prior carrier generally remains responsible for the remainder of stay unless the member elected the succeeding carrier or the prior insurer is an NY insurer without extended benefits or the prior plan is self-funded without extended benefits.
If no succeeding carrier exists, prior carrier remains responsible except for New York insurance products or self-funded plans without extended benefits.
Obtain and retain written carrier determination of financial responsibility before submitting claims or seeking prior authorization when carrier responsibility is disputed.
