This document defines UnitedHealthcare's Continuity of Care benefit interpretation, describing when members may continue care with terminated or out-of-network providers and referencing applicable federal and California state continuity statutes. It applies to UnitedHealthcare members and contracting providers in the jurisdictions covered by the document.
