Requirement for screening trial success prior to permanent implant

A percutaneous or staged screening trial demonstrating adequate symptomatic improvement is required prior to approval for permanent sacral nerve stimulator implantation for urinary or fecal indications. The screening trial must document patient selection, trial-phase response, and that the individual is capable of operating the device. Trial success is defined as improvement in reported symptoms of 50% or greater compared to baseline. For neurogenic patients, screening and selection must include documentation that symptoms are not attributable to a progressive neurologic disease (for example, Parkinson's disease, stroke, spinal cord injury, or multiple sclerosis) unless there is clear evidence supporting benefit in that specific neurogenic condition. Preoperative counseling and informed consent must be documented, including discussion of expected benefits, risks (including need for revision or removal), alternative therapies, and device operation requirements.
