inv-01: Not medically necessary / Unproven interventions

The following procedures are considered unproven and not medically necessary due to insufficient evidence of efficacy:

inv-02: Annular Closure Device (ACD) — trial-aligned criteria and findings

Covered when ALL of the following are met (evidence derived from trials summarized):

ACD candidate criteria: Patient has symptomatic radiculopathy due to lumbar disc herniation and a large post-discectomy annular defect (typically ≥ 6 mm), underwent single-level limited lumbar discectomy (L4–S1), and had failure of conservative care (commonly >6 weeks) or indication for surgery; imaging documents adequate disc height and non-osteoporotic lumbar bone mineral density as applicable to device labeling.annular defect typically ≥ 6 mm; device labeling: 4-6 mm tall and 6-10 mm wide for Barricaid

inv-03: Percutaneous injection of allogeneic cellular/tissue-based products

Covered when ALL of the following are met (if supported by sufficient evidence):

Allogeneic intradiscal injection candidate: Patient has chronic discogenic low back pain related to single- or two-level degenerative disc disease unresponsive to adequate conservative management; documented treatment plan and procedural details (product, levels, guidance) are provided. Note: randomized evidence is limited and inconsistent (e.g., Beall/VAST RCT showed no clear benefit on primary outcomes), therefore coverage requires high-quality supportive evidence specific to the product and indication.

Evidence currently insufficient to establish clear effectiveness; prior authorization and plan-level review recommended.

inv-04: Thermal Intradiscal Procedures (IDET, IDB, ID-PRF, PLN)

Covered when ALL of the following are met (evidence-based caveats):

Thermal intradiscal procedures candidate: Patient with chronic discogenic low back pain refractory to conservative care (documented), with diagnostic concordance (e.g., positive provocative discography, preserved disc height where required) and procedural details documented (technique, levels, imaging guidance). Evidence quality is heterogeneous: IDB has randomized trial support versus conventional medical management, IDET evidence is mixed, and PIRFT/discTRODE lacks benefit over sham in RCTs; therefore, coverage is limited to indications supported by high-quality evidence and consistent with study eligibility.

Randomized controlled and comparative trials vary by technique; authorization may require documentation of prior CMM and concordant diagnostic testing.

PIRFT / discTRODE - evidence summary:

For guidance on coverage of percutaneous discectomy for axial or radicular pain, see the Medical Policy titled Minimally Invasive Spine Surgery Procedures. This policy does not govern percutaneous discectomy indications or coverage determinations and directs reviewers to that separate policy for those procedures.

Percutaneous injections of allogeneic cellular/tissue-based products and thermal intradiscal procedures are not supported when used outside studied indications because current evidence is insufficient. The policy concludes there is insufficient high‑quality evidence to establish safety and efficacy for these procedures in general clinical use, and recommends that use outside rigorously studied indications should be considered unproven.

Randomized, double‑blind trial data do not demonstrate consistent benefit for some intra‑annular thermal therapies. For example, the Kvarstein PIRFT (discTRODE) trial showed no significant difference versus sham at primary endpoints and was stopped early for futility; authors concluded they would not recommend intra‑annular thermal therapy with the discTRODE probe. National guidance (NICE) similarly finds efficacy evidence inconsistent and of poor quality for percutaneous electrothermal treatments of the annulus.

This policy considers three main groups of interventions for discogenic low back pain: annular closure devices (ACDs), percutaneous allogeneic cellular/tissue‑based disc injections, and thermal intradiscal procedures (TIPs) such as IDET, IDB, and PIRFT. Each group is evaluated separately with respect to available randomized trials, systematic reviews, and guideline conclusions.

Based on the current evidence synthesis, percutaneous allogeneic cellular/tissue‑based injections and most thermal intradiscal procedures lack sufficient high‑quality evidence to support routine coverage. The policy finds these approaches are unsupported by sufficient evidence for general use and therefore are considered unproven in this context.

The randomized, double‑blind, placebo‑controlled trial of intra‑annular PIRFT using the discTRODE probe did not show benefit over sham at the trial’s primary endpoint and was terminated early when no treatment effect was observed; the study authors stated they would not recommend intra‑annular thermal therapy with the discTRODE probe.
