Medical Necessity and Coverage Criteria

Nasal valve repair — medically necessary criteria

Nasal valve procedures/repair of nasal vestibular stenosis or alar collapse are considered reconstructive and medically necessary when ALL of the following are present:

Nasal valve repair medical necessity: Prolonged, persistent obstructed nasal breathing due to internal and/or External Nasal Valve compromise; and

Other causes treated: Other causes of nasal obstruction (e.g., rhinosinusitis, allergic rhinitis, vasomotor rhinitis, nasal polyposis, adenoid hypertrophy, nasopharyngeal masses) have been adequately treated with maximal therapy and nasal obstruction persists; and

Septum/turbinate status: Nasal septal deviation and turbinate hypertrophy either are not present, have been previously surgically treated, or are scheduled to be surgically treated at the same time as the nasal valve procedure; and

Visible collapse documented: Documented evidence of visible collapse of the alar (lower lateral) cartilage (External Nasal Valve) and/or lateral nasal wall (internal nasal valve) with deep inspiration; and

Modified Cottle maneuver: Documented evidence of subjective and audible improvement in nasal airflow during a modified Cottle maneuver; and

Photographic documentation: Photos clearly document dynamic collapse of the internal and/or External Nasal Valve or anatomical deformities narrowing the internal and/or External Nasal Valve consistent with clinical examination; and

Surgical plan required: Surgeon has clearly described whether the nasal valve compromise is static or dynamic, which valve(s) are involved (internal, External, or both), and a plainly stated surgical plan including the need for a cartilage graft.all elements required

Rhinophyma excision — medically necessary criteria

Rhinophyma excision is considered reconstructive and medically necessary when ALL of the following are present:

Primary rhinophyma indication: One of the following: (a) prolonged, persistent obstructed nasal breathing due to rhinophyma; or (b) chronic infection or bleeding unresponsive to medical management due to rhinophyma; and

Photographic documentation: Photos clearly document rhinophyma as the primary cause of an anatomical Mechanical Nasal Airway Obstruction or chronic infection and are consistent with the clinical examination; and

Procedure intent: The proposed procedure is designed to correct the anatomical Mechanical Nasal Airway Obstruction and relieve the nasal airway obstruction by correcting the deformity, or the proposed procedure is designed to address the chronic infection/bleeding related to rhinophyma.all elements required

Congenital anomaly rhinoplasty

Rhinoplasty for congenital anomalies is considered reconstructive and medically necessary when performed for nasal deformity associated with congenital craniofacial anomalies (including cleft lip/palate and listed syndromes).

Rhinoplasty-primary — medically necessary criteria

Rhinoplasty-primary is considered reconstructive and medically necessary when ALL of the following are present:

Primary indications: Indication is one of: prolonged, persistent obstructed nasal breathing due to nasal bone and septal deviation as the primary cause; nasal fracture with displacement severe enough to cause obstruction; or residual large cutaneous defect after malignancy/trauma; and

Septoplasty insufficient: Nasal airway obstruction cannot be corrected by septoplasty alone, as documented in the medical record; and

Photographic documentation: Photos clearly document the nasal bone/septal deviation as the primary cause of an anatomical Mechanical Nasal Airway Obstruction and are consistent with clinical examination; and

Procedure intent: Proposed procedure is designed to correct the anatomical Mechanical Nasal Airway Obstruction and relieve the nasal airway obstruction by centralizing the nasal bony pyramid and straightening the septum; and

Rhinoplasty-revision — medically necessary criteria

Rhinoplasty-revision is primarily cosmetic but is considered reconstructive and medically necessary when ALL of the following are present:

Revision indication: Required as treatment of a complication or residual deformity from primary surgery originally performed to address a functional impairment, when a documented functional impairment persists due to the complication/deformity; and

Photographic documentation: Photos clearly document the secondary deformity/complication as the primary cause of an anatomical Mechanical Nasal Airway Obstruction and are consistent with clinical examination; and

Procedure intent: Proposed procedure is designed to correct the anatomical Mechanical Nasal Airway Obstruction and relieve the nasal airway obstruction or treat the complication; and

Symptom burden: Nasal airway obstruction is causing significant symptoms; and

Rhinoplasty-tip — medically necessary criteria

Rhinoplasty-tip is primarily cosmetic but is considered reconstructive and medically necessary when ALL of the following are present:

Tip-drop as primary cause: Prolonged, persistent obstructed nasal breathing due to tip drop that is the primary cause of anatomical Mechanical Nasal Airway Obstruction; and

Photographic documentation: Photos clearly document tip drop as the primary cause consistent with clinical examination (e.g., acute columellar–labial angle); and

Procedure intent: Proposed procedure is designed to lift the nasal tip to relieve obstruction; and

Symptom burden: Nasal airway obstruction is causing significant symptoms (e.g., chronic rhinosinusitis, difficulty breathing); and

Unproven / Not Medically Necessary

The following procedures/techniques are considered unproven and not medically necessary due to insufficient evidence of safety and/or efficacy:

Clinical selection principles

Clinical coverage should consider the following evidence and guideline statements:

Guideline and evidence context: Surgical repair of the nasal valve can improve nasal obstruction symptoms for appropriately selected patients; evaluation should identify and optimize treatment of other contributors to nasal airway obstruction and tailor the repair to involved anatomic sites.

Based on AAO-HNS position statement and systematic reviews noting benefit in selected patients.

Outcome evidence

Evidence demonstrating symptomatic and objective improvement:

Supportive outcome data: Multiple cohort studies, systematic reviews, and meta-analyses report improvements in NOSE, SNOT-20, VAS, rhinomanometry, and olfaction after functional rhinoplasty and related nasal procedures, though study designs are heterogeneous and often low level; pooled NOSE improvements of ~43–50 points at up to 12 months have been reported.

Magnitude and durability vary; higher-quality prospective trials are limited.

Evidence limitations

Limitations to consider when assessing medical necessity:

Evidence limitations: Most published studies are single-center, retrospective, small sample size, lack control groups, or are level IV case series; randomized trials are scarce, outcome measures and follow-up durations are heterogeneous, and long-term data are often lacking.

These limitations should temper strength of coverage determinations and support requirement for careful patient selection and documentation.

Coverage criteria for lateral wall implant or SSB reduction

Covered when ALL of the following are met

Primary indications: Patient has clinically significant nasal airway obstruction primarily due to dynamic lateral wall collapse (nasal valve collapse) or septal swell body hypertrophy, documented with baseline NOSE or equivalent validated PRO and clinical examination.

Prior conservative management and discussion: Documentation of prior appropriate conservative therapies where applicable (e.g., intranasal steroids, antihistamines) and shared decision-making regarding risks/benefits of implant versus surgical options.

Provider documentation of plan and outcomes: Plan includes baseline and post-procedure objective or standardized patient-reported outcomes (NOSE, VAS) at planned follow-up intervals (eg, 3, 6, 12 months) and monitoring for device/procedure-related adverse events; care plan included in prior authorization/medical record.

NST/NSB reduction — evidence summary

Evidence-based findings for nasal septal swell body/turbinate-targeted procedures:

NST/NSB evidence summary: Small prospective and retrospective studies and one RCT report short-term symptomatic and objective improvements with RFA, coblation, and microdebrider-assisted submucosal septal swell body reduction; benefits are typically assessed at 3–6 months and long-term efficacy remains uncertain.

Study limitations include small samples, nonrandomized designs, concurrent procedures, and short follow-up.

PNN / SPG ablation — evidence summary

Findings for posterior nasal nerve or sphenopalatine region ablation procedures:

PNN ablation evidence summary: Randomized trials and prospective single-arm studies report clinically meaningful reductions in rTNSS/TNSS and improved quality-of-life measures up to 12–36 months for TCRF (RhinAer) and cryoablation (ClariFix), with responder rates commonly 70–85% and generally mild transient adverse events; however many studies are industry-sponsored, heterogeneous, and have limitations including uncontrolled medication use and loss of sustained control groups.

Longer-term, independent RCT data are limited.

TCRF evidence summaries

Evidence summaries for temperature-controlled radiofrequency (TCRF) treatment of the nasal valve and related procedures:

Silvers et al. RCT (primary efficacy): Randomized trial comparing bilateral TCRF versus sham with primary endpoint responder rate at 3 months (≥20% reduction in NOSE or ≥1 clinical severity category): active arm responder rate 88.3% vs sham 42.5%; mean NOSE change -42.3 vs -16.8 (p<0.001). Three nonserious device/procedure-related adverse events reported.

Han et al. 12‑month cohort follow-up: Cohort follow-up to the RCT reports sustained NOSE improvements through 12 months with adjusted mean changes ~ -41 to -45 and a 12-month responder rate of 89.8%; no device-/procedure-related serious adverse events reported. Limitations include lack of randomized control beyond 3 months and potential confounding by medication use.

FDA clearances (informational)

FDA device clearances and classifications (informational only):

VivAer Stylus received 510(k) clearance (March 2020) as a Class II device for coagulation of soft tissue in the nasal airway to treat nasal airway obstruction by shrinking submucosal tissue in the internal nasal valve area.

Latera Absorbable Nasal Implant received 510(k) clearance (June 23, 2016) indicated to support nasal upper and lower lateral cartilage (PLLA‑PDLA copolymer); informational only and not a basis for coverage decisions.

ClariFix and RhinAer: ClariFix (cryoablation) cleared Feb 14, 2017 (Class II) for destruction of tissue including use in chronic rhinitis; RhinAer Stylus cleared July 29, 2022 (Class II) for destruction of soft tissue in the nasal airway including posterior nasal nerve regions.

Procedures that do not meet the prescriptive reconstructive criteria set forth in this policy are not considered reconstructive and therefore may be denied. Examples include rhinoplasty (primary, revision, or tip) and rhinophyma excision performed for purely cosmetic reasons or without the required documentation that the deformity is the primary cause of mechanical nasal airway obstruction. The policy lists specific checklist-style requirements (e.g., documented visible collapse on deep inspiration, modified Cottle maneuver response, photographic documentation, and a clear surgical plan) that must all be satisfied for nasal valve repairs to be considered medically necessary; failure to meet any element results in a determination of not medically necessary. Surgery that corrects an anatomical congenital anomaly but does not improve or restore physiological nasal function is classified as cosmetic and excluded from reconstructive coverage. Older studies of nasal septal swell body (NST/NSB) procedures that used cautery or cryosurgery did not demonstrate a clear benefit. For example, Haight et al. found no symptom or rhinometry differences at 10–16 weeks between groups receiving inferior turbinate reduction with or without NST reduction using cautery/cryosurgery. Follow-up in many NST studies was short (commonly 3–6 months), limiting conclusions about durability. Given these limitations, the policy does not support routine coverage of those older cautery/cryosurgery NST techniques as effective alternatives.
