Unproven/Not Medically Necessary Procedures: Axial Lumbar Interbody Fusion (AxiaLIF); Percutaneous Image-Guided Lumbar Decompression (PILD); Percutaneous sacral augmentation (Sacroplasty); Automated Percutaneous Lumbar Discectomy (APLD); Percutaneous Endoscopic Lumbar Discectomy (PELD); Minimally Invasive Lumbar Decompression (MILD); Laparoscopic Anterior Lumbar Interbody Fusion (LALIF); Transforaminal Lumbar Interbody Fusion (TLIF) using only endoscopy visualization.

AxiaLIF (Axial lumbar interbody fusion)

Coverage stance determined by insufficient high-quality comparative evidence:

Evidence summary: No randomized controlled trials evaluating AxiaLIF for L5–S1 conditions were identified; evidence consists primarily of case series and retrospective studies at high risk of bias, and comparative effectiveness versus standard interbody fusion approaches is unclear. Improvement in net health outcomes, complication and failure rates, and need for further surgery have not been demonstrated to be comparable or superior to standard techniques.

Laparoscopic and robotic ALIF (LALIF, R-ALIF)

Coverage stance based on limited and heterogeneous evidence:

Evidence summary: Published evidence for laparoscopic ALIF (LALIF) and robotic ALIF (R-ALIF) is largely prospective and retrospective case series and nonrandomized trials, many older in date; pooled reviews indicate LALIF yields similar outcomes to mini-open ALIF but with longer operative times, higher technical demand and a potentially higher complication rate. Data for R-ALIF are promising but limited and heterogeneous; further high-quality studies are needed before concluding superiority or equivalence.

Minimally Invasive Lumbar Decompression (MILD/PILD)

Covered with criteria when clinical indications and prior treatments align with studied populations:

Patient selection criteria: Diagnosis of lumbar spinal stenosis with symptomatic neurogenic claudication primarily due to hypertrophied ligamentum flavum; age ≥ 18 years; failure of conservative management (e.g., physical therapy, medications) and/or prior epidural steroid injections as per clinical trial protocols prior to consideration of MILD/PILD.

Derived from trials and consensus guidelines included in systematic reviews and RCTs.

Evidence and duration: Randomized and nonrandomized trials and systematic reviews report statistically and clinically significant improvements in pain and function through 1–2 years with relatively low adverse event rates (reported adverse event rate ~8.2%), but long-term durability beyond 2 years and comparative effectiveness versus open decompression remain uncertain.

Percutaneous laser disc decompression

Not reliably supported by current evidence:

Evidence summary: Health technology assessments judge percutaneous laser disc decompression evidence very limited and low quality; an RCT showed noninferior pain outcomes versus conventional surgery but substantially higher reoperation rates (e.g., 38% vs 16% in Brouwer), and uncertainty remains regarding long-term comparative effectiveness and need for subsequent surgeries.

Percutaneous Image-Guided Lumbar Decompression (PILD / mild®)

Percutaneous Sacroplasty

Percutaneous sacroplasty: Evidence for sacroplasty in sacral insufficiency fractures varies by etiology: meta-analyses show high technical and clinical success for osteoporotic or malignant sacral fractures (technical success ~98.9%, clinical success ~95.7%), but overall evidence is poor for non-malignant causes and no randomized trials exist; Hayes and guideline reviews conclude minimal or unclear support for sacroplasty in many SIF contexts and recommend further RCTs.

Chandra meta-analysis supports osteoporotic/malignant SIF use; Hayes notes limited support for other causes.

Percutaneous / Endoscopic Lumbar Fusion

Current data are insufficient to establish long-term comparative safety and effectiveness versus established fusion techniques; further well-designed RCTs are needed.

General evidence findings: Multiple meta-analyses and retrospective studies report that endoscopic, biportal, percutaneous, and other minimally invasive variants of TLIF and MIS-TLIF have similar medium- to long-term outcomes, with some short-term advantages (less blood loss, shorter hospitalization, earlier recovery) but often longer operative times; pooled analyses are limited by heterogeneity, small sample sizes, and nonrandomized designs.

When newer endoscopic techniques may be considered: Consider endoscopic or percutaneous fusion variants when reduced intraoperative blood loss or shorter hospitalization are clinically important and when surgical expertise and informed patient selection are present; weigh these potential short-term benefits against longer operative time and limited high-quality evidence for long-term outcomes.

Evidence quality caveat: Many studies are retrospective, single-center, or small and have risk of bias; systematic reviewers and evidence assessments call for well-designed randomized trials before broad conclusions of superiority; coverage decisions should account for limited evidence quality.

The following spinal procedures are designated as unproven and not medically necessary because current evidence is insufficient to establish clinical benefit: Axial Lumbar Interbody Fusion (AxiaLIF), Percutaneous Image-Guided Lumbar Decompression (PILD), Percutaneous sacral augmentation (sacroplasty), Automated Percutaneous Lumbar Discectomy (APLD), Percutaneous Endoscopic Lumbar Discectomy (PELD), Minimally Invasive Lumbar Decompression (MILD®), Laparoscopic Anterior Lumbar Interbody Fusion (LALIF), and Transforaminal Lumbar Interbody Fusion (TLIF) performed using only endoscopic visualization.

Society guideline guidance is reflected in exclusions: for patients with isolated lumbar stenosis without deformity or instability, fusion is not recommended because it has not been shown to improve outcomes compared with decompression alone (AANS/CNS, 2014). Additionally, NASS concludes there is insufficient evidence to support cost-effectiveness or broad endorsement of minimal-access fusion or interspinous spacing devices in certain contexts, and the evidence base does not currently support routine use of these fusion/spacer technologies.

The evidence for percutaneous sacroplasty is limited and of low quality. Hayes and other reviews report insufficient support for sacroplasty in sacral insufficiency fractures from non‑osteoporotic, non‑malignant causes, and recommend larger randomized trials to establish safety and long‑term effectiveness. For select osteoporotic or malignant sacral fractures, published series and meta-analyses report high technical and clinical success but randomized comparative data are lacking, so broader coverage is not established.

Where evidence is limited to small, single‑center, retrospective studies or very low‑quality reviews (for example, some endoscopic transforaminal and other percutaneous fusion system evaluations), these procedures may be excluded or restricted until higher‑quality comparative data (well‑designed randomized controlled trials) are available to demonstrate safety and durable effectiveness.

The policy revision removed several procedure definitions from the definitions section. Removed items include Interlaminar Lumbar Instrumented Fusion (ILIF), Nucleoplasty, Percutaneous or Endoscopic Lumbar Fusion, Translaminal (TESSYS®), Interlaminar Endoscopic Surgical Systems, and Tubular Retractor.

AxiaLIF and multiple other minimally invasive spine procedures lack adequately powered randomized controlled trials comparing them to standard surgical approaches. For AxiaLIF specifically, no RCTs demonstrating comparative safety and effectiveness for L5–S1 conditions were identified, and available case series and retrospective studies carry a high risk of bias; therefore the evidence is insufficient to conclude that these techniques provide comparable or superior clinical outcomes.

Several procedures have an insufficient evidence base to conclude a net health benefit. Examples include AxiaLIF (no RCTs showing comparative benefit for L5–S1), percutaneous laser disc decompression (limited, low‑quality trials with higher reoperation rates), and certain indications for PILD/mild® where long‑term durability and comparative effectiveness versus open decompression remain uncertain.

For Percutaneous Image‑Guided Lumbar Decompression (PILD / mild®), the literature includes randomized and nonrandomized studies but is limited by unblinded trials, variable follow‑up, and risk of bias. Hayes and other reviews determined that while short‑term symptom and function improvements up to 1–2 years have been reported, the overall evidence is low quality and insufficient to establish long‑term benefit or superiority over open decompression.

When the published evidence consists primarily of low‑quality, nonrandomized, single‑center series or studies at high risk of bias (as noted in ECRI and other assessments for some endoscopic and percutaneous systems), coverage may be denied or limited pending higher‑quality comparative trials that address patient‑oriented outcomes and longer‑term durability.

Clinical Background and Rationale

Back pain and radiculopathy related to disc herniation are common and frequently managed initially with conservative care. Surgical decompression is generally considered when neuropathic pain persists despite several months of conservative management. Minimally invasive techniques have been developed to reduce procedural morbidity, but for a number of these approaches the evidence does not yet demonstrate clear, durable advantages over standard surgical care; therefore conservative therapy and documented failure of nonoperative management should be considered before pursuing novel procedures.
