Background & Evidence

Transpupillary thermotherapy (TTT), also called diode laser hyperthermia, uses a diode laser to raise temperature within tumor tissue, producing heat-induced sclerosis of vascular channels and tumor regression. It has been studied primarily for ocular tumors such as choroidal melanoma and retinoblastoma, and has also been investigated for choroidal neovascularization, age-related macular degeneration (AMD), and other ocular conditions.

Evidence supports use of TTT for selected small choroidal melanomas and as adjunctive therapy in some settings: published series and randomized/controlled studies report tumor regression and variable recurrence rates, and guidelines (NCCN) note a role for TTT in small recurrences or when radiation/surgery are not feasible. For retinoblastoma, prospective and observational studies report good tumor control and eye-preservation rates when TTT is used as primary or adjunctive therapy in selected small tumors.

For other indications (for example CNV/AMD and various other ocular conditions), clinical trial data and systematic reviews are insufficient to demonstrate a vision benefit or consistent safety profile. Randomized trials show reduced anti-VEGF injection frequency with low-dose TTT but no improvement in visual acuity, and guideline bodies (NICE, AAO) have concluded evidence is inadequate or no longer recommend thermal laser for subfoveal CNV. Overall, evidence is insufficient to support TTT as medically necessary for these other indications.
