Eligibility criteria: Patient is greater than 12 months of age; diagnosis of a confirmed biallelic RPE65 mutation-associated retinal dystrophy (e.g., Leber's congenital amaurosis, retinitis pigmentosa, early onset severe retinal dystrophy); genetic testing documenting biallelic RPE65 mutations; sufficient viable retinal cells as determined by OCT confirming an area of retina within the posterior pole of > 100 µm thickness; prescribed and administered by an ophthalmologist or retinal surgeon experienced in providing sub-retinal injections; patient has not previously received Luxturna treatment in the intended eye; provider does not request a planned inpatient admission for the sole purpose of administering Luxturna; authorization limited to no more than one treatment per lifetime per eye and authorization valid for no longer than 45 days from approval.

Coverage for Luxturna (voretigene neparvovec-rzyl) is limited to patients who are older than 12 months. The FDA guidance and the policy specify that treatment is not recommended for patients younger than 12 months because retinal cells are still undergoing proliferation and the delivered vector and transgene product could be diluted or lost during that period. This age threshold is required to ensure adequate chance of durable treatment effect and is applied when assessing medical necessity and prior authorization requests.

Authorization is limited to one lifetime treatment per eye. Requests for repeat treatment of an eye previously treated with Luxturna (beyond one treatment per lifetime per eye) are not supported as medically necessary under this policy. Background

Leber’s congenital amaurosis (LCA) and autosomal recessive retinitis pigmentosa (RP) are inherited, early-onset retinal dystrophies characterized by photoreceptor dysfunction and progressive vision loss. Biallelic mutations in the RPE65 gene impair the visual cycle and lead to loss of retinal pigment epithelium–dependent photoreceptor function.

RPE65-associated disease may be amenable to a one-time gene augmentation therapy (Luxturna) when sufficient viable, post-mitotic retinal cells remain. Viability is assessed by the treating clinician using optical coherence tomography (OCT); the policy defines sufficient viable retinal cells as an area within the posterior pole with retinal thickness greater than 100 µm. Because retinal cells continue to proliferate in infancy, treatment is not recommended for patients younger than 12 months.
