Saphnelo is medically necessary for the treatment of moderate to severe systemic lupus erythematosus (SLE) when ALL of the following criteria are met:

Diagnosis of moderate to severe systemic lupus erythematosus, without severe active central nervous system lupus or severe active lupus nephritis
Concomitant therapy: Currently receiving at least one standard of care treatment for active systemic lupus erythematosus (e.g., antimalarials, corticosteroids, or immunosuppressants) that is not a biologic
Benlysta requirement: Patient has a contraindication, intolerance, or failure to Benlysta (for Medicare reviews, refer to the CMS section*)
No combination biologic: Patient is not receiving Saphnelo in combination with a biologic agent or Benlysta
Dose per FDA label: Saphnelo is dosed according to US Food and Drug Administration labeled dosing for SLE
Instruction in 'Instructions for Use' section
Prescriber: Prescribed by or in consultation with a rheumatologist

Headline efficacy results: In the Phase IIb study, anifrolumab 300 mg achieved an SRI(4) response at week 24 of 34.3% versus placebo 17.6% (p=0.014). In pooled TULIP-1/2 analyses, anifrolumab was associated with a lower annualized flare rate (rate ratio 0.75, 95% CI 0.60–0.95) and prolonged time to first flare. In TULIP-2, the BICLA response at week 52 was anifrolumab 47.8% versus placebo 31.5% (difference 16.3 percentage points; p=0.001). Limitation: these efficacy evaluations did not include patients with severe active lupus nephritis or severe active CNS lupus.

Background & Definitions

Background: Saphnelo is a human IgG1κ monoclonal antibody that binds subunit 1 of the type I interferon receptor (IFNAR) to antagonize IFNAR and inhibit type I IFN signaling, including inducing internalization of IFNAR1 and reducing downstream IFN-responsive gene expression and inflammatory processes. Approximately 60–80% of adult patients with active SLE express elevated type I IFN–inducible genes. The FDA indication is for treatment of adult patients with moderate to severe SLE who are receiving standard therapy. The efficacy of Saphnelo has not been evaluated in patients with severe active lupus nephritis or severe active CNS lupus; use is not recommended in these situations.
