Admission Eligibility

Covered when ALL of the following are met

Home Health Admission Criteria: The patient must be confined to his/her home; must need skilled nursing on an intermittent basis, physical therapy, speech-language pathology services, or continued need for occupational therapy; must be able to have needs met by the agency's stated scope of service; must be under the care of a primary care physician licensed to practice medicine or osteopathy in the state of Nevada who will order and approve the provision of services; must be located within the geographic area served by SMA Home Health (direct services in metropolitan Las Vegas; patients outside the area may be served via contractual arrangements and services may be limited); the home environment must be safe and able to meet the patient's physical needs and ensure staff safety; services and care must conform to current standards of practice for the respective discipline; the patient's clinical condition must be within the feasibility of the agency's services with respect to time, cost, personnel and skills; acceptance is based on the patient's willingness and ability to function in a non-institutional environment; eligibility is not based on protected characteristics; patients who cannot be cared for by the agency will be referred to other resources; if the patient does not meet admission criteria the referring physician will be promptly notified.

If the patient’s clinical condition is outside the feasibility of the agency’s services — including limitations of time, cost, personnel, skills, or the agency’s stated scope of service — the patient will not be admitted or will be discharged. Similarly, a home environment that is not safe or cannot meet the patient’s physical needs and ensure staff safety precludes admission or continuation of services. Services must be able to be provided safely and effectively in the patient’s home; when they cannot, discharge occurs and the attending physician will be notified and required to document pertinent information in the medical record.

Acceptance for home care is also contingent on the patient’s willingness and ability to function in a non‑institutional environment; when these conditions cannot be met, the agency will redirect care or refer the patient to appropriate alternative resources.

Provider Responsibilities, Orders, and Admission Process

Service capacity requirement

Acceptance of a referral is contingent on the agency's ability to meet the patient's needs within the agency's stated scope of service, available time, cost, personnel and skills; if the agency cannot provide required services, the referral may be redirected or the patient referred to other resources.

The attending physician must order and approve provision of services and, upon physician-ordered discharge, all pertinent information regarding the patient's condition must be documented in the patient's medical record.

Referrals may be declined if the patient is outside the agency's geographic service area (direct services limited to metropolitan Las Vegas), if the home environment is unsafe or cannot meet the patient's physical needs, or if the patient's clinical condition is not within the feasibility of the agency's services; patients who cannot be cared for will be referred to other resources and the referring physician will be notified if admission criteria are not met.
