Initial therapy for Alhemo (concizumab-mtci) is permitted when ALL of the following conditions are met:

Alhemo initial therapy criteria: Based on clinical patient assessment, the provider has determined that the patient is not an appropriate candidate for Hympavzi (document reason) OR patient is currently receiving Alhemo therapy.

This criterion was added to medical necessity criteria effective 01/01/2026.

Esperoct is designated as not medically necessary for treatment of hemophilia A for the following uses: routine prophylactic treatment, perioperative management of surgical bleeding, and treatment of bleeding episodes. The policy rationale states that published clinical evidence does not demonstrate superiority in efficacy or treatment adherence of Esperoct compared with other available recombinant factor VIII products.

Ixinity and Rebinyn are listed as not medically necessary for hemophilia B for routine prophylaxis, peri‑operative management, and treatment of bleeding episodes. The policy notes that published clinical evidence does not show superiority of these agents versus other available recombinant factor IX products and uses this as the basis for the non‑coverage determination.
