inv-04: Infantile hemangioma — conditional coverage for laser (evidence & guideline considerations)

Covered when criteria below are met:

ALL of the following

Infantile hemangioma (IH) or cutaneous hemangioma that is symptomatic or functionally impairing (e.g., causing bleeding, ulceration, airway compromise, or visual axis obstruction).
Pulsed dye laser (PDL) or other laser modality is used as an adjunct or secondary therapy after consideration of, or trial of, established first‑line pharmacologic management (e.g., oral propranolol) unless there is a documented contraindication to pharmacologic therapy or the lesion is judged by a hemangioma specialist to require earlier laser intervention.
Treatment is delivered in consultation with a hemangioma specialist (or appropriate pediatric/dermatology specialist) with documentation of the clinical rationale for laser use, including lesion characteristics, prior therapies and expected benefit.

inv-05: Pilonidal sinus disease — conditional coverage for laser epilation (LHD/LE) — ALL of the following

Covered when ALL of the following are met:

ALL of the following

Diagnosis of pilonidal sinus disease (PSD) with history of recurrent disease or as adjunctive therapy after surgical management when hair regrowth in the sacrococcygeal region is contributing to recurrence or ongoing symptoms.
Laser hair depilation (LHD) or laser epilation (LE) is provided in addition to standard care measures (e.g., wound care, hygiene education, and instruction/supplies for regular hair removal) — documentation must show the patient received or was offered standard care education and supplies prior to or concurrent with laser intervention.
For surgical candidates, LHD/LE is performed as adjunctive therapy to reduce hair regrowth after definitive surgical treatment or per ASCRS guidance when used as primary or adjunctive hair removal in the absence of abscess.
Provider documents the chosen laser type, treatment regimen (number and spacing of sessions), and clinical rationale with expected benefit and follow-up plan.
