Coverage criteria for D6081
SelectHealth will reimburse D6081 when documentation demonstrates the following findings on chart notes and a current periodontal chart:
ALL of the following
- Bleeding upon probing
- Inflamed gingival tissue
- Large pocket depths (4mm or more)4mm or more
And/or
- Discomfort at the site
