Services are reimbursed when coded and documented in accordance with CMS/NCCI guidance and Priority Health documentation requirements; certain services are bundled and not separately payable unless specific conditions are met.

Coverage and documentation prerequisites

Coding and modifier use must follow CMS/NCCI guidance and Priority Health documentation requirements.
Providers are responsible to review coding rules and apply modifiers where appropriate and supported by documentation.
Documentation must include wound details (size of area to be debrided, drainage, condition of surrounding tissue, texture, temperature and any additional specific elements identified during the assessment) when debridement is performed.

Bundling and separate reimbursement rules

Dressing changes performed as part of debridement or other wound care are considered components of the procedure or E/M and will not be reimbursed separately.
Casting, splinting and strapping (including Unna boot and multi-layer compression systems) are included in the surgical procedure when performed with debridement and are not separately payable unless performed at a separate location and supported by documentation.
Whirlpool services may be separately reimbursed when performed on the same date of service as nonselective debridement or for services unrelated to wound care.
Whirlpool services are not separately reimbursed when selective debridement is performed on the same date of service unless two separate wounds are treated using separate modalities; documentation must support distinct service modifiers.

Modifier and separate-procedure operational notes

Modifier application is appropriate only if the service is performed at a separate location and supported by documentation as guided by CMS NCCI chapter 4.
Use modifiers XE, XS, XP or XU to provide greater specificity instead of modifier 59 when appropriate; use modifier 59 only if no other specific modifier applies.
Documentation will be requested when Unna boot (29580) or multi-layer compression dressing (29581) are coded with a wound debridement on the same date of service; documentation must support use of distinct service modifiers for separate wound/modality situations.

Nonselective debridement (97602): documentation must clearly describe techniques utilized (e.g., abrasion, enzymatic) and wound assessment details.
Surgical debridement (11042–11047): follow CMS area increment guidance (first 20 sq cm or less; additional units per each additional 20 sq cm or part thereof).

Authorization requirements and medical necessity appropriate to the procedure, diagnosis and frequency must be obtained. An authorization does not guarantee payment if billing, coding or policy requirements are not followed.

Documentation Required

Provide documentation when coding Unna boot or multi-layer compression with debridement

Priority Health will request documentation when Unna boot (29580) or multi-layer compression dressing (29581) are billed on the same date as a wound debridement; documentation must support use of distinct service modifiers and that the service was performed at a separate location when applicable.

Documentation must support use of distinct service modifiers for separate wound/modality situations.
Modifier XE/XS/XP/XU should be used instead of modifier 59 when appropriate; modifier 59 only if no other specific modifier applies.
Casting/splinting/strapping codes shall not be reported for application of a dressing after a therapeutic procedure unless documentation shows a separate location/service.
