Coverage is conditional and subject to documentation, setting, and frequency limits described below.

ALL of the following

Sleep studies must be performed by a certified sleep lab facility and interpreted by a certified sleep specialist; home-based studies must be billed with the appropriate home place of service (12).
Overnight stays that occur during testing do not change inpatient status and are considered part of the sleep study (not separately reimbursed).

ALL of the following

Complete and thorough documentation to substantiate the procedure performed is the responsibility of the provider; consult applicable guidelines and the Provider Manual for any specific documentation requirements.

ALL of the following

No more than two polysomnography sessions are reimbursable for diagnosis or modification of a treatment plan within any 12-month period.
Polysomnography used to titrate CPAP therapy is payable for a single session; routine polysomnography for CPAP titration is not separately payable.

ALL of the following

One home sleep study is payable per 12-month period; additional sessions require supporting documentation demonstrating medical necessity.

ALL of the following

Up to three sleep nap sessions will be reimbursed when diagnosing narcolepsy.
Multiple sleep latency studies include multiple sessions but should be coded as a single unit.

ALL of the following

CPT 95803 (actigraphy) encompasses recording, analysis, interpretation, and report over the monitoring window; do not report CPT 95803 more than once in any 14-day period.
Do not report CPT 95803 in conjunction with CPT 95806–95811.

ALL of the following

CPT 95810 and 95811 should not be billed separately for the same night; bill the appropriate code based on the service performed in accordance with AMA, NCCI, and CMS guidance.

Reimbursement rates are located in payer fee schedules; listing of codes is for reference and inclusion does not guarantee coverage or payment—benefit coverage is determined by the member's plan and applicable law.
Priority Health may perform pre- or post-claim reviews and may reject, deny, or recoup payments for noncompliance with policy or regulatory requirements.

Coverage will be considered for services furnished in the appropriate setting to the patient's medical needs and condition; authorization may be required per the Provider Manual.
For Medicare indications that do not meet NCD, local LCD, or specific medical policy criteria, a Pre-Service Organization Determination (PSOD) is required; CMS and MDHHS guidelines apply unless otherwise specified.

Pre‑service determination (PSOD) required for certain Medicare indications

For Medicare: if the requested indication does not meet a National Coverage Determination (NCD), local LCD, or a specific Priority Health medical policy, a Pre‑Service Organization Determination (PSOD) must be completed. See the Provider Manual for PSOD instructions and workflow.

A PSOD is required for Medicare indications lacking applicable NCD/LCD or specific medical policy guidance.

Billing Rule

Authorizations (pre‑service determinations) do not guarantee payment. Providers must submit accurate, supported coding and documentation and follow place‑of‑service and facility requirements. Sleep studies must be performed in certified sleep labs and interpreted by certified sleep specialists; home studies must use the appropriate home place of service code. Overnight stays associated with sleep testing are not separately reimbursed. Priority Health may perform pre‑ or post‑claim payment integrity reviews and may deny, recoup, or reject claims that do not meet billing, coding, or documentation standards.
