Covered when documentation confirms malnutrition and provides sufficient evidence to support coding to the level of severe malnutrition.

Documentation confirms presence of malnutrition and validates severity to support coding to highest specificity.

ALL of the following

Treatment protocols must match the severity reported on the claim (treatment plan for underlying etiology present).
Clinical documentation should describe inpatient nutritional support and planned post-discharge support.
Clinical indicators and contributing factors are documented (e.g., inadequate intake, impaired absorption, increased requirements, inflammation, social/environmental factors).
Any additional patient complexities or contributing etiologies are detailed in the medical record.

Diagnostic frameworks accepted

ASPEN criteria: at least TWO of the following six characteristics - insufficient energy intake; weight loss; loss of muscle mass; loss of subcutaneous fat; localized or generalized fluid accumulation that may mask weight loss; diminished functional status (e.g., handgrip strength).
GLIM criteria: combination of at least ONE phenotype (nonvolitional weight loss, low BMI, or reduced muscle mass) AND ONE etiologic criterion (reduced food intake/absorption or inflammation due to acute disease/injury or chronic disease).

Laboratory and anthropometric thresholds may be used to support severity: albumin < 2.0 g/dL and/or prealbumin < 5 mg/dL; usual body weight < 75% with unintended weight loss > 5% in 1 month, > 7.5% in 3 months, > 10% in 6 months, or > 20% in 1 year.

Timeframe and illness context documented: acute illness/injury versus chronic illness (consider role of acute vs chronic inflammation) and social/environmental factors affecting intake or self-care.

Billing impact noted: documentation must support coding to E40–E43 when severe malnutrition criteria met; accurate documentation required to support DRG assignment and to avoid denials or recoupment.
