Coverage criteria for infusion supplies and pumps

Claims must comply with code-to-pump associations, bundling rules, frequency limits, and CMS documentation standards. Non-compliant coding will be denied.

All of the following must be satisfied for claim payment:

Code-to-pump associations
- Supply HCPCS codes A4224 and A4225 must be billed only with insulin external infusion pump E0784; use with any other pump will be denied as incorrect coding.
- Supply HCPCS codes A4221, A4222, and K0552 must be billed only with non-insulin external infusion pumps (E0779, E0780, E0781, E0791, or K0455); billing these supplies with an incompatible pump will be denied.
- For pumps with multiple associated supply groupings (E0748, E0791, K0455, etc.), suppliers must bill only supply codes listed as 'Associated codes' for the specific pump configuration; use of codes listed as 'Non-associated codes' will be denied.
Bundling and unbundling
- A4224 describes all necessary supplies for administration with E0784 (continuous subcutaneous insulin); separate billing for items included in A4224 is unbundling and will be denied.
- Codes A4230 and A4231 are included in A4224 and are not valid for separate claim submission.
Compounded drugs and ancillary items
- Compounded drugs reported with J7999 billed with an external infusion pump will be denied.
- Compounded drugs that do not use code J7999 will be denied as incorrect coding when billed with an external infusion pump.
- Replacement battery codes K0601–K0605 are not separately payable when billed with a rented infusion pump.
Multiple visits on same date
- If multiple visits occur on the same date of service, suppliers must bill only one visit and should report the highest paying visit with the applicable drug; claims reporting multiple visits on the same line date of service will be returned.
Frequency limits (claim-level frequency compliance)
- Adhere to the following HCPCS/S-code frequency limits
  - A4230 and A4231: 31 per month.
  - A4232: 60 per month.
  - E0784 (insulin external infusion pump): 1 per 5 years; 10 per 5 years when billed with Modifier RR.
  - S9374, S9375, S9376, S9377 (select S-codes) : 210 per year.
  - S9490–S9504 (select S-codes): 60 per 4 months.
Documentation and authorization
- Claims must meet CMS standard documentation requirements for supplies and DME; see CMS Article A55426 for required documentation.
- Authorization, when obtained, does not guarantee payment; proper coding, billing, and documentation must still be met and noncompliance may result in denial or recoupment.

Priority Health's billing policies guide accurate claim submission and define coding and reimbursement requirements, but an authorization or determination of medical necessity does not ensure reimbursement if documentation, coding, billing or contractual requirements are not met. Authorization requirements and medical necessity remain subject to review and must align with procedure, diagnosis, frequency, and applicable policy. Payment integrity reviews (pre- or post-claim) may be performed to validate billing and coding accuracy and to prevent fraud, waste, and abuse. If government program regulations, policy rules, or contractual obligations are not followed, Priority Health may reject or deny the claim and recover or recoup payments. An authorization on file does not supersede coding, billing, or reimbursement requirements.

Priority Health may perform pre- or post-claim payment integrity reviews and may recover/recoup payments if requirements are not met.
