Incidental, Included and Non-Reimbursable Items

Non-reimbursable incidental and included items

Priority Health does not reimburse separately for services and supplies that are incidental, integral, mutually exclusive, included in room and board or facility charges, excessive/inappropriate, or personal convenience items. The lists below are illustrative and not all‑inclusive.

Non-reimbursable when billed with a more comprehensive primary service

Types of non-reimbursable items/services
- Mutually exclusive procedures: services or procedures that cannot reasonably be performed on the same member on the same date of service during the same surgical session based on procedural definition or anatomical considerations.
- Incidental services: services clinically integral to the expected outcome of a primary service, including technical charges for purchase, rental and/or maintenance of equipment; these are not separately payable when performed with a more comprehensive primary service.
- Integral procedures: services included within HCPCS/CPT code–defined procedures (some components have separate CPT codes only when not performed as part of another procedure).
- Global allowance: Evaluation & Management (E&M) reporting rules per Medicare Global Surgery Rules apply to E&M services with procedures covered by those rules.
Common categories and examples of items considered packaged (not eligible for separate reimbursement)
- Disposable supplies furnished in inpatient or outpatient settings that cannot be reused (examples: syringes, bandages, garter belts, sheaths, bags, blood or urine testing supplies, needles, masks, gauze, replacement batteries, stockings).
- Contaminated, wasted or unused items: Items opened in error, items not utilized during the encounter, supplies prepped but not used, items wasted due to technical failure or canceled cases (examples include unused DME, crash cart items, emergency drugs).
- Basic hospital room daily charges: Equipment, services and supplies associated with personal care, nursing or technical personnel, and room furnishings across settings (ED, critical care, observation, treatment rooms, cath lab, surgical suites, neonatal, recovery, etc.); routine medical supplies, equipment and devices are inclusive and should not be separately reported.
- Routine medical supplies (illustrative list): Alcohol swabs; all peripheral and central line supplies; blood gas kits; baby powder; band-aids; basin; bedpans; blood tubes; cotton balls; personal care items (deodorant, razors, shampoo, soap, toothbrush/toothpaste, lotion, socks, slippers); drapes; emesis basin; masks/gloves/gowns; skin cleansing liquids; heating light/pad; ice packs; needles; oral swabs; oxygen masks; PICC line supplies; prep kits; restraints; linens/blankets/pillows; saline/irrigation solutions; sharps containers; telemetry/interactive devices; specimen collection tools; Kleenex; lubricant jelly; meal trays; syringes; tape; thermometers; toilet paper; tongue depressors; urinals.
Operational and billing notes
- Claims are reviewed at the line level to detect incidental or component services billed separately; revenue codes submitted for routine supplies/equipment/nursing care that are components of room and board charges may be denied or recouped.
- The lists above are illustrative and not all‑inclusive; items determined to be excessive or personal convenience items are also not separately reimbursable.

Providers and facilities are responsible for accurate and complete documentation and accurate billing and coding; claims not following guidelines will be rejected and must be rebilled or adjusted.

An authorization is not a guarantee of payment. Authorization requirements and medical necessity appropriate to procedure, diagnosis and frequency are still required, and an authorization on file does not supersede coding, billing or reimbursement requirements. If government program regulations, policy details, or contractual requirements aren't followed, Priority Health may reject or deny the claim or recover/recoup claim payment.

Providers and facilities are responsible for accurate and complete documentation and for submitting claims using industry-standard, compliant CPT, HCPCS and revenue codes that reflect services actually performed and fully documented. Claims not following these billing or documentation requirements will be rejected, denied, or subject to recoupment and may require rebilling or adjustment.

Document services/items in the medical record to the highest level of specificity before reporting CPT/HCPCS/revenue codes.
