Routine foot care — definition and scope of services

DefinitionRoutine foot care includes treatment of calluses, corns, keratotic and hyperkeratosis lesions, keratoderma, bunions (except capsular or bone surgery thereof) and nails (except surgery for ingrown nails).

Onychomycosis — definition and testing context

DefinitionOnychomycosis (mycotic nails) is a fungal infection of the nail plate, nail bed, or both.

Clinical presentationsMay present as distal subungual (nail thickening, yellowing, keratin/debris accumulation), proximal subungual, or white superficial infections (chalky white scale beneath the nail surface).

EtiologyCan be caused by dermatophytes (ringworms) or other fungi such as yeast.

Debridement of hyperkeratotic lesions (CPT 11055-11057) is not separately reimbursable when performed with nail debridement (CPT 11720-11721) on the same digit per National Correct Coding Initiative (NCCI) guidance; a modifier will not override this edit.

Nail debridement codes (CPT 11720-11721) are payable when performed without hyperkeratotic lesion debridement on the same digit and submitted with required documentation; not payable when performed with hyperkeratotic lesion debridement on the same digit.

Modifiers should follow CMS NCCI guidance; modifiers may be applied only when supported by documentation. A significant and separately identifiable service on the same day as routine care may be submitted with modifier 25. Incorrect modifier use may result in denials.
