Coverage criteria and billing rules

Coverage is contingent on medical necessity, appropriate documentation, adherence to coding/modifier guidance, and specified frequency and technical rules.

ALL of the following

Clinical history, reason for the study, and examination findings must be documented (preferably written by the referral source).
Numerical data gathered during NCS (latency, amplitude, conduction, etc.) must be recorded and available — preferably in a tabular format; hard copy of waveforms or a complete written report with interpretation must be submitted upon request.
Document muscles tested, presence/type of spontaneous activity, characteristics of voluntary unit potentials and interpretation.
Record type of needle used (monopolar or concentric).
Name, signature, and professional designation of all individuals performing, interpreting, or supervising the test must be included.

ALL of the following

ANY of the following
- Narrative reports stating 'normal' or 'abnormal' without numerical data.
- Description of F-wave without corresponding motor conduction data.
- Pattern-setting unilateral H-reflex measurements.
- Absence of clinical history supporting the need for the test.
- Lack of documentation to support repeat testing on the same beneficiary or routine testing of every patient referred for pain.

ALL of the following

Frequency limits: Up to 2 tests per year for diagnoses such as carpal tunnel, radiculopathy, mononeuropathy, polyneuropathy, myopathy, and neuromuscular junction disorders; up to 3 tests per year for motor neuron disease, plexopathy, and acute inflammatory demyelinating conditions; additional testing may be allowed with justification.
Nerve conduction studies reimbursed only once per nerve or named branch regardless of number of sites or methods used on that nerve (motor and sensory testing considered separate).
Nerve conduction tests performed with preconfigured electrodes customized to a specific anatomic site are payable only once per limb and cannot be billed with other nerve conduction codes for that limb.

E/M services are usually included in the exam immediately before and during EMG/NCS; if an E/M is separate and identifiable, medical record must document medical necessity and the E/M should be billed with modifier 25.
Apply standard billing and coding guidelines (including CMS NCCI). Modifiers (e.g., 59, XE, XS, XU, 25) should be used only when supported by documentation; incorrect modifier use may result in denials.

EMG definitionElectromyography (EMG) are diagnostic tests used to assess the electrical activity of muscles.

NCS definitionNerve conduction studies (NCS) assess the function of peripheral nerves and must provide real-time response parameters including amplitude, latency, configuration and conduction velocity.

Data requirementsPriority Health requires NCS to provide real-time numerical parameter data; delayed interpretation or transmitted raw measurements without real-time parameters are not acceptable substitutes.
