Priority Health will reimburse professional ECG interpretation when documentation and reporting requirements are met; certain scenarios are not payable.

ALL of the following

Member demographics.
A complete, written report similar to one prepared by a specialist that addresses relevant clinical issues, available comparative data, and test findings; the report format must be separately identifiable and may be a separate heading in the clinical record or written on the ECG tracing with a reference in the chart.
Indication that the reporting provider personally performed the interpretation.
Legible signature of the provider of service and date of interpretation.

When interpreting a computer-generated report, documentation must state agreement or disagreement with the computer findings and note any findings disagreed with or additional/corrected findings.

Not payable in the following situations

Claims with overly general language (e.g., 'ECG normal') — considered review only and not payable.
ECG performed for screening or as part of a routine physical examination without associated signs, symptoms, or complaints — not payable.

Multiple/professional component readings: Generally only one professional component (PC) is paid; a second PC may be paid only when additional physician expertise is necessary, reasonable, significantly greater than the first reader, and substantially contributes to the interpretation (for example to clarify a questionable finding).

Modifier use: Apply modifiers (e.g., 76, 77) per CMS/NCCI guidance and only when documentation supports their use.

Billing and Authorization Expectations

Billing Rule

Only bill ECG interpretation when complete written report and documentation exist

Submit professional ECG interpretation claims only when a complete written report and supporting documentation are present in the medical record. The report must include member demographics, a complete written report addressing relevant clinical issues and comparative data, indication the reporting provider personally performed the interpretation, legible provider signature, date of interpretation, and documentation demonstrating medical necessity. Authorization and medical necessity requirements per the member’s benefit plan and applicable government program rules still apply; an authorization is not a guarantee of payment and claims may be denied or recouped if billing, coding, or documentation requirements are not met.

Do not submit CPT 93000, 93010 or G0403 unless the written interpretation/report and medical record fully document the service.
Ensure the report is separately identifiable (separate heading, paragraph, or noted on the ECG tracing) and includes provider signature and date.
Follow applicable authorization and medical necessity rules; an authorization does not guarantee payment.
Failure to document services rendered to the required level of specificity may result in denial, rejection, or recovery of payment.
