Diagnostic testing requirements: The diagnostic test used to establish OSA must be an FDA‑approved diagnostic device; the test results must meet coverage criteria applicable to the date of service; the test must be ordered by the treating practitioner; and the test must be conducted by a qualified provider in accordance with state regulations.

Custom oral appliance (E0486) device qualifications: Custom mandibular advancement devices billed as E0486 must have a fixed mechanical hinge, provide adjustable mandibular protrusion, retain settings and position during sleep, and require no ongoing dental adjustments beyond 90 days.

Non‑DME / non‑covered devices: Devices considered dental and not covered as DME include TMJ oral appliances (D7880), tongue retaining devices for OSA/snoring, appliances provided for snoring without an OSA diagnosis, devices for other dental conditions, and appliances that require adjustments beyond 90 days; such items should be coded as A9270 when non‑covered or as the appropriate dental code.

Surgical services coverage statement: Home testing, sleep studies, CPAP and other treatments (including certain surgical procedures such as UPPP) are covered when their specific medical necessity criteria are met; surgical procedures may be subject to non‑standard cost‑sharing depending on the member's plan.

Obstructive sleep apnea services coverage

The following items are not covered as durable medical equipment because they are considered dental in nature: TMJ oral appliances, tongue retaining devices for OSA or snoring, appliances provided solely for snoring without an OSA diagnosis, devices intended to treat other dental conditions, and appliances that require ongoing dental adjustments beyond 90 days.

An authorization on file does not supersede proper coding, billing, or reimbursement requirements. Providers must follow CPT, HCPCS, revenue code and other applicable coding guidelines and document services to the highest level of specificity; lacking appropriate documentation or correct coding may result in denial, recovery, or recoupment of payment.

Replacement of oral appliances for normal wear-and-tear is allowed only after 5 years of use. Early replacement before 5 years for ordinary wear-and-tear is not covered; exceptions are limited to loss, theft, or irreparable damage (for example, accident, fire, or flood).
