Protective helmets intended for sports or recreational activities (for example, bike or ski helmets) are explicitly excluded from coverage and are not a benefit under this policy. These items are considered routine protective equipment rather than medically necessary orthotic devices.

Replacement of a cranial helmet or orthosis due to loss or accidental damage is not a covered benefit. Coverage is limited to items that meet medical necessity criteria; routine replacement for loss or damage is the member’s responsibility.

The HCPCS code S1040 (Cranial remolding orthosis, pediatric, rigid, with soft interface material, custom fabricated, includes fitting and adjustment(s)) is identified in the policy coding list but is specifically noted as not payable for Priority Health Medicare. Helmet therapy is not generally indicated as the first-line treatment for infants with mild to moderate positional skull deformity. Helmets are primarily considered for severe deformity or when conservative measures fail.
