inv-01: Monitoring Indications

Monitoring indications: A plasma HIV RNA baseline level may be medically necessary in any patient with confirmed HIV infection.

Periodic monitoring: Regular periodic measurement of plasma HIV RNA levels may be medically necessary to determine risk for disease progression and to determine when to initiate or modify antiretroviral treatment regimens.

High-risk/anticipated therapy situations: In clinical situations where the risk of HIV infection is significant and initiation of therapy is anticipated (e.g., persistence of borderline or equivocal serologic reactivity; signs and symptoms of acute retroviral syndrome such as fever, malaise, lymphadenopathy, and rash in an at-risk individual).

Coverage for quantification CPT codes is limited to claims billed with the following diagnoses

Diagnosis linkage: CPT codes 87536 (HIV-1 quantification) and 87539 (HIV-2 quantification) are covered when billed with one of the following ICD-10 codes: B20, B97.35, R75, or Z21.

Viral quantification is not indicated as the primary diagnostic tool for initial HIV detection. For diagnostic or screening purposes, qualitative assays, antigen/antibody combination tests, or other recommended diagnostic methods should be used; quantification is reserved for establishing baselines and ongoing monitoring, and coverage for CPT codes 87536 and 87539 is limited to claims billed with appropriate HIV-related ICD-10 codes.

Services must be performed and fully documented in the medical record to the highest level of specificity. Use industry-standard CPT, HCPCS, and revenue codes consistent with the service performed. Documentation should support the codes and level of service billed, including clinical findings, rationale for the service, and any supplies or devices provided.

If government program regulations, policy, or contractual requirements are not followed, Priority Health may reject or deny the claim and recover payments.
