This policy (Policy No. 91651) defines medical necessity criteria used to determine when a procedure should be performed in a Hospital Outpatient Department (HOPD) rather than an Ambulatory Surgical Center (ASC) or physician office. It applies to site-of-service determinations only and does not set medical necessity for individual procedures. The policy excludes emergency services and inpatient procedures. Status: CURRENT.
