The actual service(s) rendered must be reported using the appropriate CPT or other HCPCS procedure code(s).

This policy applies to claims submitted on CMS-1500 paper claim forms or 837P electronic claim forms and to all Premera Blue Cross, Premera Blue Cross Blue Shield of Alaska, LifeWise Health Plan of Washington, LifeWise Assurance Company, and Premera Blue Cross HMO lines of business and products.
