Covered when ALL of the following statements are acknowledged:

- HCPCS codes S9083 and S9088 are informational and not reimbursed by the Plan whether billed alone or with other services.
- The actual service(s) rendered must be reported with an appropriate CPT or HCPCS procedure code and Place of Service code 20 (Urgent Care Facility).
- Exceptions to this policy may be made where a provider or employer group contract dictates otherwise.
- Violations of this policy may result in increased auditing and monitoring, contractual penalties, or termination of the contract at the Plan's discretion.
